WELCOME | Michigan Employees

Helping you and your family reach and maintain good health is very important to Smile Partner's. Smile Partner's is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Smile Partner's effective the first of the month following one month of full time employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and child(ren). Please view your benefit booklet for details on when coverage ends due to eligibility.


Note: If your spouse is eligible for group medical insurance through their own employer and you elect to cover your spouse on this medical coverage, your will be subject to a $50 per month surcharge in addition to your employee contributions.

How To Enroll In Benefits

ENROLLMENT INSTRUCTIONS:

  1. Review your Benefit Guide and determine the best plan options for you and your family
  2. Sign in through www.workforcenow.adp,
  3. Make your elections through the Workforce Now portal
  4. Submit elections
  5. Keep a copy of your confirmation and benefit elections for your records


The benefit choices you make during your new hire enrollment remain in effect for the remainder of the 2022 benefit year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Smile Partner's provides those employees who are covered by one of the BCBSM/BCN medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

DID YOU KNOW?

  • Eye exams can help detect serious eye and general health conditions sooner: high blood pressure, diabetes, heart disease, high cholesterol
  • Babies should receive their first professional eye exam at 6 months
  • 80% of learning in the first 12 years comes through the eyes


Your vision coverage is provided by EyeMed, through Unum. This coverage is provided to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Review your coverage details in your Benefit Guide.

Smile Partner's provides you with financial protection options for life, accidental death & dismemberment, short term disability and long term disability with Unum.


Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Smile Partner's provides all eligible employees with a $25,000 benefit. Employees also have the opportunity to enroll in voluntary Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death.


All new eligible employees may enroll up to the guarantee issue amount with no evidence of insurability. If you do not enroll at this time you will be required to submit Evidence of Insurability for Unum to review and determine whether you will be approved for coverage at a later date.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Please note, if you do not elect coverage at the time of your new hire enrollment, you will be subject to evidence of insurability if you choose to enroll at a later date.


Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Please note, if you do not elect coverage at the time of your new hire enrollment, you will be subject to evidence of insurability if you choose to enroll at a later date.

FSA

Flexible Spending Accounts

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


You may contribute as follows:

Health Care FSA

  • Up to $2,850 annually
  • $570 maximum carryover of unused funds
  • All benefit eligible can participate
  • If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses


Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns

Health Savings Account

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.


To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

  • Covered by a Qualified High Deductible Health Plan
  • You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
  • You cannot be covered by Medicare or Medicaid
  • You cannot be eligible to be claimed as a tax dependent
  • You have not received VA benefits in the past 90 days


HSA IRS Maximums are as follows:

$3,650 for a single contract

$7,300 for a two person or family


$1,000 "Catch Up" contribution for those age 55+

Smile Partner's provides all employees and their dependents access to the Employee Assistance Program with Unum. For those employees covered by a BCBSM/BCN medical plan, you and your covered dependents also have access to virtual mental health services with BCBSM Online Visits (registration link included in Telemedicine section); you also have access to the MyStrength program which will asses your mental health needs as well as provide daily tips and resources to improve the areas of your mental health where you need it most!
