Your Benefit Guide

Helping you and your family reach and maintain good health is very important to Modern Builders Supply. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Who is Eligible?

Employees working at least 30 hours per week are eligible to participate in benefits offered by

Modern Builders Supply. Most of your benefits are effective on the 90th day following your date of hire.


Generally, you may only change your benefit elections during the annual enrollment period. However, you may change your benefit elections during the year if you experience an event such as:

  • Marriage, divorce or legal separation
  • Birth of your child
  • Death of your spouse or dependent child
  • Adoption of or placement for adoption of your child
  • Change in employment status of employee, spouse or dependent child
  • Qualification by the Plan Administrator of a child support order for medical coverage
  • Entitlement to Medicare or Medicaid

At Modern Builders Supply we want employees to be engaged in their jobs and communities, be active and engaged in their physical health and ultimately achieve their highest level of well-being. Our wellness program is designed to:

 

•Provide healthy lifestyle education, resources and support to employees

•Help you lead a healthier life

•Manage health care costs for all of us

 

You (and your covered spouse) must complete an annual physical in order to pay reduced medical premiums. If you (and your covered spouse) fail to complete an annual physical by December 31, 2023, your medical premium increases by 20% beginning in 2024 – additionally, if enrolled in the HSA plan, the company deposit of $625 (employee only coverage) or $1,250 (family coverage) for 2024 will not be made.

 

Annual Physical—Participate in an annual physical exam by a licensed physician by December 31, 2023. The physician just needs to sign the Physician Examination Form(s) (included in the Appendix) confirming that the employee/spouse completed the required examination. Remember that the Affordable Care Act mandates that preventive care be covered 100% (no deductible). Make sure to tell your physician that you are only having a wellness visit so that your visit is no cost for the physical.

 

Health Coaching—If you have a health condition, you have access to your own personal nurse coach through Meritain Health Nurse Health Coaching program. Your nurse coach will help you understand and manage your condition to start living a healthier lifestyle. Pre-Admission and Post-Discharge Counseling Meritain offers a Pre-Admission and Post-Discharge Counseling service. With the Pre-Admission Counseling, you can speak with a case manager prior to an inpatient elective surgery. During the call, case managers educate you on proper preparation for your hospital admission and recovery process. They discuss medications, go over signs and symptoms of infection, and send you additional educational materials, if necessary. During the Post-Discharge Counseling, case managers will call you to check on your health status and assess your pain or discomfort levels. They identify any complications or additional needs. Case managers also discuss with you when to contact your physicians or seek medical attention. More information will be provided on how to get started in January.

 

Tobacco Cessation Meritain offers a Nicotine Free program from Healthy Merits to its members to help smokers become tobacco free. You will have access to an online program and live health coaching. There is no cost to you. More information will be provided on how to get started in January. Express Scripts also provides up to a 180-day supply of tobacco cessation products to members at no charge per year (i.e., Buproprion, Nicotrol Nasal Spray, Nicotrol Inhaler, Nicotine Kits, Nicotine patch, Nicotine gum, Nicotine lozenge, and Chantix). To find an Express Scripts participating pharmacy, go to https://www.express-scripts.com or call Customer Service at 800.334.8134.

How to locate network providers

To find physicians and medical facilities in Meritain’s PPO network, you may contact Meritain’s Customer Service at 1.800.343.3140 or by accessing the website below. Networks differ based on where you live, please refer to your Identification card to determine which network is primary for you.

24/7 Nurse Line

Meritain provided a Nurse Line available to you 24 hours day, 7 days a week. You can speak to a nurse abut advice for a health concern. If you have questions about treating minor medical conditions at home, help you understand medical tests and results, etc. Call the nurse line at 1.866.726.6529.


Are you interested in a high deductible health plan?

The HDHP is similar to the PPO Plan in that you have the option to choose any provider when you need care. However, in exchange for a lower per-paycheck cost, you must satisfy a higher deductible that applies to almost all health care expenses, including those for prescription drugs.

All expenses are your responsibility until the deductible is reached, with the exception of preventive care, which is covered at 100% when you visit a physician in the network. Once the deductible is met, you are responsible for the plans’ coinsurance for medical expenses and prescription drug expenses.

Enrolling in this plan allows you to contribute tax free dollars to a health savings account (HSA) in addition to the contributions that are made by Modern Builders Supply. Any dollars that you and Modern Builders Supply wish to contribute can be used towards any eligible medical, Rx, dental and vision expenses that you may incur while covered under the plan. See the HSA section of this guide for additional details.


Preventive Services Covered at 100%

All plans recognize routine preventive services at 100%, no coinsurance, no deductible as long as the claim is submitted as “routine or preventive” and the services performed falls within the approves list of preventive services. For a complete and updated listing, please visit https://www.uspreventiveservicestaskforce.org.

During your wellness visit, proactively let your physician know the reason for the appointment is for a wellness visit and that your physician needs to submit and code the visit as routine, preventive in nature. If your visit is submitted with a diagnosis, the wellness visit will not be paid at 100%, but instead, will be subject to deductible and coinsurance.  Below are a few examples of services that can be recognized as preventive:

•Routine Wellness Exams, including well baby & child routine exams

•Cholesterol and lipid level screening

•Pelvic exam, pap test and screening mammograms

•Colorectal cancer screening, colonoscopies, sigmoidoscopies (age limit applies)

•Vaccines & immunizations: Hepatitis A & B, Influenza, Pneumonia, Shingles

•Contraceptives (specific list applies)

•Diabetes screenings

Modern Builders Supply provides 3 plan options through Aetna/Meritain

If the provider is in network, there is a higher level of coverage which means lower out of pocket costs to you. If the provider is non-network, a lower level of coverage which results in higher out of pocket costs to you. You choose which plan best fits your needs based on your utilization of services and contribution costs.

is your Pharmacy Benefit Manager and you have a dedicated concierge service called RxBenefits that will provide one-on-one assistance, guidance and authorization help.


TRADITIONAL DRUGS

TIER 1 (GENERIC) | Lowest copay: Most drugs in this category are generic drugs. Members pay the lowest copay for generics, making these drugs the most cost-effective option for treatment.

TIER 2  | Higher copay: This category includes preferred, brand name drugs that don't yet have a generic equivalent. These drugs are more expensive than generics, and a higher copay.

TIER 3  |  Highest copay: In this category are nonpreferred brand name drugs for which there is either a generic alternative or a more cost-effective preferred brand. These drugs have the highest copay. Make sure to check for mail order discounts that may be available.

Dental

Dental Plan

Modern Builders Supply’s dental program is administered by Meritain Health, a subsidiary of Aetna. You are free to receive care from any dentist, but you will save money if you see dentists within Aetna’s network.


PREVENTION FIRST!

Your dental health is an important part of your overall health. Make sure you take advantage of your preventive dental visits.

Preventive care services are covered at 100% if you visit an In-Network provider. They are also not subject to the annual deductible.

Modern Builders Supply offers vision coverage through VSP to help pay for eye exams, prescription glasses, and contact lenses. You receive a higher level of benefits when you see a provider in the VSP Network. Click the link below to find a provider in your area.

BASIC LIFE INSURANCE

Life insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. AD&D insurance is equal to your Life benefit in the event of your death being a result of an accident, and may also pay benefits for certain injuries sustained.

SHORT-TERM DISABILITY (STD)

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. 

Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.


Cost of Coverage: Voluntary Benefit

Employee is responsible for 100% of the cost


Elimination Period

This is the number of days that must pass between your first day of a covered disability & the day you can begin to receive your disability benefits.

Benefits begin on the 1st day of an accident and the 8th day of an illness (including pregnancy)


Benefit Duration

Payments may last up to 13 weeks

You must be sick or disabled for the duration of the waiting period before you can receive a benefit payment.


Coverage Amount

Covers 60% of your weekly income, up to a maximum benefit of $500 per week.


What's covered?

A variety of conditions and injuries.

Typical claims would include: pregnancy, injuries, joint, back and digestive disorders.


401k

You may also be eligible to participate in Modern Builders Supply 401(k) Profit Sharing Program.

 

The 401(k) plan allows an employee to make pretax contributions to the plan at commencement of employment on a weekly basis. After an employee has completed six months of service, he/she will be eligible to receive a 20% match on any contributions on the next available entry date (either January 1 or July 1).
