Helping you and your family reach and maintain good health is very important to LightGuide Inc. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. LightGuide Inc remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. Highlights of the 2025-26 benefit plans include:

Eligibility & Enrollment
All FULL-TIME employees working at least 30 hours per week are eligible to participate in the benefit plans offered by LightGuide Inc. Newly hired employees will be eligible the 1st Day of full-time employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse, domestic partner and children.
Children are eligible for benefits as follows:
•Medical, Dental and Vision: Until the last day of the year in which the child turns 26
Action Required: All elections must be submitted by November 7, 2025!
This is an ACTIVE ENROLLMENT! All employees must access Rippling and either elect or waive benefits by November 7, 2025; otherwise, all benefits (with the exception of employer paid Life/AD&D, STD, & LTD) will be terminated effective 11/30/2025.
ENROLLMENT INSTRUCTIONS:
- Review your benefit information included on this site
- Log into Login - Rippling
- Follow instructions and enroll in your benefits - you must complete open enrollment even if you are waiving benefits
- Make sure to save your elections and save/print your confirmation statement.
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Blue Cross Blue Shield of MI (BCBSM) & Blue Care Network (BCN) will continue to be your medical and prescription carrier. You have the choice between three plans for the 2025-26 plan year. The three plan options include a PPO plan a POS 1000 plan and a POS HSA plan. View the Providers & Networks for more information on how these plans networks differ from each other as well as buttons to locate participating providers.
If you are already enrolled in one of the LightGuide Inc BCBSM/BCN medical plans today and have not already done so, please be sure to register through the BCBSM member portal where you can access important information pertaining to your medical plans with BCBSM including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. If you are not currently covered, you will be able to register in the BCBSM portal as of your effective date of coverage. Information on how to register can be found here.

Look up your prescriptions by viewing the BCBSM Drug List. Use the Control F feature to type in the name of your prescriptions and determine which level the drug is and whether a Prior Authorization (PA) or Quantity Limits (QL) apply. You can also determine whether there may be lower cost alternatives for your prescriptions.
Helpful Rx Cost Saving Tools & Tips!
MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. You can get more information on how to start receiving mail order drugs by clicking here.
GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).
ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.
If you seek care out of network, you will experience higher cost sharing in addition to possible balance billing. View the comparison below for additional details on the differences between the PPO and POS, plans when it comes to networks and access.

- PPO PROVIDERS Enter your zip code and select "Yes this is Correct" - then in the upper right corner select your plan "PPO Plans"
- POS MI PROVIDERS Use this button if you are a MI Resident electing a POS Plan or a non-MI resident traveling to MI. Enter your zip code and select "Yes this is Correct" - then in the upper right corner select your plan "Blue Elect Plus POS"
- POS NON-MI PROVIDERS Use this button if you live outside of MI or are a MI resident traveling outside of MI. Enter your zip code and select "Yes this is Correct" - then rather than entering your card information, select "Browse a list of plans" and select "Blue Elect Plus POS"

Members will continue to get the same urgent and behavioral health virtual care that they do today but with a new look and feel with better integration with other virtual care solutions they may be engaged in.
All eligible members will need to download the Teladoc Health app and create a new account with Teladoc Health if you have not done so already.
Virtual Urgent Care
Have a virtual visit with a U.S. board-certified doctor for minor illnesses such as colds, sore throats, urinary tract infections and pink eye. Visits are available for adults and children 24/7.
Mental Health Services
Through the Mental Health option, you can connect with a licensed therapist or U.S. board-certified psychiatrist when you’re dealing with stressful situations or issues such as grief, anxiety and depression. Mental health visits require an appointment, but many therapists and psychiatrists have evening and weekend availability.

Accounts
You have the opportunity to contribute to a variety of savings and/or spending accounts on a pre-tax basis which lowers your taxable income. View the information below for important details and guidelines for the various accounts offered by LightGuide Inc.
The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. The POS Medical Plan is qualified for an HSA. By contributing to an HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
HSA IRS Maximums are as follows and are on a calendar year basis. The maximums indicated below are a combined maximum for participant and any employer provided funds to the HSA. If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.
For the 2026 Tax Year:
$4,400 for a single contract | $8,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+
The Flexible Spending Account (FSA) allows you to set aside pre-tax dollars to pay for eligible medical and/or dental and vision expenses. The nice part about an FSA is you have access to the full annualized amount on day one!
The election you make cannot be changed throughout the year unless the change aligns with the qualifying event. For example, if you get married, you can increase your contribution, however, you cannot decrease or end your FSA for this type of life event.
There are two types of FSA options:
- General Purpose FSA: To pay for qualifying medical, dental, and vision expenses. This type of FSA is only available if you do not have an HSA.
- Limited Purpose FSA: To pay for qualifying dental, vision, and post-deductible medical expenses. This type of FSA is available for those who have an HSA.
Plan Details are as follows:
- You can contribute up to $3,200 annually.
- At the end of the calendar year, if you have not used all of your FSA funds, there will be no carry over into next year's FSA plan (should you elect an FSA for the 2026 plan year).
- The plan includes a Grace Period which allows you to incur and submit expenses using your 2024-25 funds until March 15, 2026.
- You have 90 days after the end of the calendar year to submit for expenses that occurred on or before 12/31/2025.
- If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only which can be used for qualifying dental, vision, and post-deductible medical expenses.
- You have access to your full annualized election amount on day one.
The Dependent Care Reimbursement Account (DCRA) allows you to set aside pre-tax dollars to pay for eligible dependent care expenses. Qualifying expenses include daycare, nursery & preschool tuition, summer day camps, and more!
The election you make cannot be changed throughout the year unless the change aligns with the qualifying event. For example, you can increase your contribution if you add another member to your family through birth or adoption, however, you cannot decrease your contribution for this type of life event.
Plan Details are as follows:
- You can contribute up to $5,000 annually or limited to $2,500 if you are married and file separate tax returns.
- The plan includes a Grace Period which allows you to incur and submit expenses using your 2024-25 funds until March 15, 2026.
- You have 90 days after the end of the plan year to submit for expenses that occurred on or before 12/31/2025.
- You only have access to the funds of which you have paid into the account.
- Any unused funds are forfeited.

LightGuide Inc's dental plans provide comprehensive coverage to help you and your family maintain good dental health. The dental plans include both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled in one of the LightGuide Inc Unum Dental plans today and have not already done so, please be sure to register through the Unum Dental member portal where you can access important information pertaining to your dental plan with Unum Dental including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Unum Dental portal as of your effective date of coverage.
You have the choice to purchase a dental plan - The plan includes a deductible (for basic and major services only), ortho coverage for dependents (through age 19), and 100% covered preventive visits.

LightGuide Inc offers vision coverage through Unum to help pay for eye exams, prescription glasses and contact lenses.
If you are already enrolled in one of the LightGuide Inc's Unum vision plan today and have not already done so, please be sure to register through the Unum member portal where you can access important information pertaining to your vision plan with Unum including participating providers, coverage information, and explanation of benefits/claims. Financial Protection
LightGuide Inc understands the importance to you and your family to have strong financial protection. We provide all employees Basic Life/AD&D and Disability coverage at no cost to you. In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, employee paid basis so you can select what is best for your family. All of these coverages are provided to protect your income and out of pocket expenses.

A sudden accident or death can leave you or your loved ones in a vulnerable position. LightGuide Inc provides all eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) coverage in the amount of 1 time's your annual salary up to $100,000 at no cost to you. Eligible spouses receive a $5,000 life benefit and eligible children receive a $1,000 life benefit if 6 months or younger and a $2,000 life benefit if 6 months or older.
You also have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in voluntary employee coverage, this will be in addition to your employer coverage.

- Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

LightGuide Inc provides both Short & Long Term Disability benefits at no cost to you!
Everyday illnesses or injuries can interfere with your ability to work, more serious issues can impact your ability to work indefinitely. Even a few weeks away from work can make it difficult to manage household costs. Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
Voluntary Benefits

Accident & Hospital coverage is provided through UNUM. Hopefully you are never in the position to use it, but if you were to experience an accident or hospitalization, it could end up costing you a lot of money. With these plans, you will be paid money for qualifying situations/services to cover our of pocket expenses or you may use the funds for whatever you like.
