There are three times when you can enroll in/or change your benefits:

Every year Lift Brands reviews the benefits package and conducts a benefit survey to gather your feedback in order to ensure the benefit options available meet your needs and provide the best combination of coverage, network access, ease of use, and affordability.

Benefits Eligibility
As a full-time employee, you are eligible for benefits the first of the month following 30 days of employment for most benefits.
In addition, the following family members are eligible for certain benefits as well:
- Your legal spouse. This does not include a common-law spouse or ex-spouse.
- Your eligible domestic partner, who is a person of the same or opposite sex with whom you have established a domestic partnership.
- Your (or your spouse/domestic partner's) biological child, stepchild, a child for whom legal guardianship has been awarded, and who is under age 26.
- Dependent child who is physically or mentally disabled and dependent on you.
Documents needed to provide proof of eligibility include:
- Birth certificate
- Marriage certificate
- Legal guardianship documents
Qualifying Life Events
If you have a qualifying life event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the qualifying life event.
Common life events include:
- Marriage, divorce, or legal separation
- Birth or adoption of a child
- Death of a spouse or qualified dependent
- Change in a child's dependent status
- Change in employment status or in coverage under another employer-sponsored plan

Benefits Enrollment
During your enrollment period you will make decisions on all of your benefits through Paylocity.
If you are a new hire or have recently become eligible for benefits and do not enroll when you are first eligible, you will only receive coverage for Basic Life/AD&D, and Short Term Disability benefits. You will be able to enroll in the other benefits at the next Open Enrollment period, unless you experience a qualifying life event.

Open Access

Lift Brands offers three medical options providing coverage for preventive care, doctor’s visits, hospitalization, prescription drugs and more.
What is a PPO Plan?
A PPO plan is a traditional medical plan in which you pay upfront copayments for certain services such as office visits or prescription drugs, and coinsurance for emergency room, outpatient, and inpatient hospital services. If performed as part of an office visit, the copay covers the cost of the visit and any lab work. However, x-rays and diagnostic imaging will be subject to the deductible and coinsurance. Preventive care is covered at 100% in-network.
What is a High Deductible Health Plan?
A High Deductible Health Plan (HDHP), is a medical plan paired with an HSA that allows you to save pre-tax money for qualified medical expenses. Under a HDHP, you pay for medical services out-of-pocket until you meet your deductible. Once the deductible is met, the plan typically covers a significant portion of your medical expenses through coinsurance. Preventive care is covered at 100% in-network, without needing to meet the deductible. Contributions can be made to the HSA, and the funds can be used for a wide range of medical expenses, including prescription drugs, dental, and vision care. Unused funds roll over year to year and can be invested for future medical expenses, including those in retirement.

Traditional Drugs
Generic | Lowest copay:
A generic drug contains the same chemical substance as a drug that was originally protected by chemical patents. Members pay the lowest copay for generics, making these drugs the most cost-effective option for treatment.
Formulary Brand | Higher copay:
This category includes preferred, brand name drugs that don't yet have a generic equivalent. These drugs are more expensive than generics, and require a higher copay.
Non-formulary Brand | Highest copay:
This category includes non-preferred brand name drugs for which there is either a generic alternative or a more cost-effective preferred brand. These drugs have the highest copay. Check for mail order discounts that may be available.
Specialty Drugs
Specialty drugs are used to treat certain complex health conditions. These drugs tend to be very expensive and may need special handling and monitoring.
Pharmacy Benefits
Employees who have elected medical coverage receive prescription drug coverage through HealthPartners. You can purchase prescriptions through a national network of retail pharmacies or through the mail order program.
Helpful Rx Cost Savings Tools & Tips:
MAIL ORDER - Many drugs are available in a 93-day supply, rather than the 31-day retail supply. Typically, you will pay less if you choose to get a mail order 93-day supply.
GOOD Rx - There are many tools online you can use to save on prescription costs. One being www.GoodRx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (will not count towards your maximums).

A full listing of covered drugs can be found at www.healthpartners.com/preferredrx. A drug list, also called a formulary, is a list of generic and brand-name drugs covered by our health plan. Review the plan materials for details on specific benefits.
You can use the drug list to see if a medication is covered by your health insurance plan. You can also find out if the medication is available as a generic, needs prior authorization, has quantity limits, and more.

Can I use my HSA to pay for qualified medical expenses for a spouse or dependent?
Yes, even if your spouse or dependent is covered under another health plan. It’s important to keep in mind that only expenses for tax dependent expenses can be reimbursed from the HSA.
Tax-Free Times Three. An HSA has triple tax savings. When the funds are used to pay for qualified medical expenses or you do not pay taxes on contributions, earnings, withdrawals.

Benefits you Receive
There are many benefits of using an HSA, including the following:
It is portable. The money in your HSA is carried over from year to year and is yours to keep, even if you leave the company.
It is a tax-saver. HSA contributions are made with pre-tax dollars. Since your taxable income is decreased by your contributions, you’ll pay less in taxes. If you use the money for qualified expenses, you don’t pay taxes when you withdraw the money either.
Reimbursement. You can get reimbursed for eligible expenses at any time once your HSA account is open as long as you have the funds available.
2026 Maximums
- $4,400 for individual coverage
- $8,750 for family coverage
Additionally, if you are age 55 or older, you may make an additional “catch-up” contribution of $1,000.
You may change your contribution amount at any time throughout the year as long as you don’t exceed the annual maximum.
Who is Eligible and When?
To be eligible for an HSA, you must be enrolled in the $6,350-100% HSA or $3,400-80% HSA plan.
In order to contribute to an HSA, you must not:
- Be claimed as a dependent on someone else’s tax return.
- Have other health plan coverage that provides benefits covered under the high deductible plan (for example, a healthcare Flexible Spending Account (FSA), a separate prescription drug plan, or a spouse’s non-HDHP).
- Have a spouse with a healthcare FSA that could reimburse your medical expenses.
- If you are covered by a healthcare FSA, it must be a limited purpose FSA, covering only vison and dental expenses.
- Be enrolled in a government health plan, such as Medicare, Medicaid, or TRICARE.

Understanding Your FSA
"Use It" or "Lose It"
You must use all the funds in your FSAs (excluding the $680 rollover amount for the healthcare and limited purpose FSA) during the 2026 calendar year, or funds will be forfeited so it is important to plan carefully. Once the plan year begins, you cannot change your election unless you have an eligible qualifying life event.
Run Out Period
The healthcare FSA, limited purpose FSA, and dependent care FSA allow a 90-day run out period for eligible expenses. The run out period is the 90 days into the new year where you have time to submit eligible claims for your FSA and get reimbursed.
For example: if you see a dentist on December 27th, 2025 and don’t get your bill until January 14th, 2026 you are able to submit that claim in 2026 and get reimbursed from your 2025 FSA.
Eligible Expenses
A full list of qualified FSA expenses can be found in IRS Publication 502 at www.irs.gov.
Eligible Dependent Care Expenses Include:
- Care for your dependent child who is under the age of 13 that you can claim as a dependent on your federal tax return;
- Care for your dependent child who resides with you and who is physically or mentally incapable of caring for themselves; or
- Care for your spouse, parent, or grandparent who is physically or mentally incapable of caring for themselves and spends at least eight hours a day in your home.
Care is defined as: In-home babysitting services (not by an individual you claim as a dependent); care of a preschool child by a licensed nursery or daycare provider; before and after-school care; summer day camp (provided it is not overnight); and in-home dependent daycare.

Eligibility
For full-time employees Lift Brands offers a plan for medical expenses, a plan for dependent care expenses and a limited-purpose plan for those contributing to an HSA medical plan.
The healthcare FSA gives participants in the $1,000-$25 copay plan the opportunity to set aside pre-tax dollars to pay for qualified medical, dental, and vision expenses. Examples of eligible expenses include deductibles, copays, prescription drug costs, and over-the-counter medicines.
Participants in the $6,350-100% HSA and $3,400-80% HSA plan can contribute to a limited purpose FSA that can be used to pay for dental and vision expenses ONLY.
Healthcare FSA
Maximum Annual Contribution | $3,400
You are eligible to participate in the healthcare FSA if you are enrolled in the $1,000-$25 copay plan. All eligible healthcare expenses – such as deductibles, medical and prescription copays, dental expenses, and vision expenses can be reimbursed from your healthcare FSA account.
With the Healthcare FSA, you can spend up to the full amount of your annual election as soon as your account has been set up.
Unused funds up to $680 will be rolled over for you to use in 2027, any amount above the rollover limit will be forfeited.
Limited Purpose FSA
Maximum Annual Contribution | $3,400
If you open or contribute to a Health Saving Account (HSA), you may only enroll in a limited purpose FSA.
If you enroll in a HDHP (High Deductible Health Plan) you will only be allowed to elect the limited purpose FSA.
A limited purpose FSA will reimburse you for dental and vision expenses, but you cannot claim the same expense from both your FSA and HSA.

Dependent Care FSA

The dependent care FSA allows you to pay for eligible dependent care expenses with tax-free dollars so that you and your spouse can work or attend school full-time. Unlike the healthcare FSA, funds in a dependent care FSA are only available once they have been deposited into your account and you cannot use the funds ahead of time.
You may set aside up to $7,500 annually in pre-tax dollars, or $3,750 if you are married and file taxes separately from your spouse.
If you participate in a dependent care FSA, you cannot apply the same expenses for a dependent care tax credit when you file your income taxes.

Life and Accidental Death and Dismemberment Insurance

You can’t always predict – or control – your life.
But you can prepare for it.
Life insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. Accidental death and dismemberment (AD&D) insurance is equal to your life benefit in the event your death is the result of an accident and may also pay benefits for certain injuries sustained.
Basic Life and AD&D Insurance
Lift Brands provides eligible employees with a $25,000 life and AD&D benefit administered by Sun Life. Lift Brands pays for the full cost of this benefit—meaning you are not responsible for paying any monthly premiums. AD&D insurance pays you and your beneficiaries if you lose a limb or die in an accident. Your maximum AD&D benefit is $25,000, but the amount received will depend on the severity of injuries as a result of an accident. Refer to the Lift Brands life and AD&D SPD for details.
Voluntary Life and AD&D Insurance
In addition to the employer paid life and AD&D insurance, employees have the option to elect additional coverage for themselves, their spouse, and children through Sun Life. When you enroll in this benefit, you pay the full cost of coverage through post-tax payroll deductions. Evidence of Insurability (EOI) may be required if you are enrolling outside of your initial eligibility period.

What will my beneficiary receive?
In the event that death occurs: Your life insurance coverage amount is paid to your beneficiary(ies) on file.
If the death occurs from an accident: 100% of the AD&D benefit would be payable to your beneficiary(ies) in addition to your life insurance.
Naming a beneficiary is a crucial part of electing life insurance. Don't forget to update your beneficiary details in Paylocity if you experience a life event, such as a divorce or birth of a child.
Open Enrollment Rules
New Hire Enrollment
During your initial enrollment period you can elect up to the guarantee issue amount without completing medical health questions, or medical underwriting. If you waive this coverage now, but elect during our annual enrollment period, you and your spouse (if electing spousal coverage) will be subject to medical underwriting.
2026 Open Enrollment
If you are currently enrolled and would like to increase your election, you may do so with medical underwriting and coverage will not take effect until Sun Life approves your application.
If you initially waived coverage and would like to enroll in coverage now, you will be subject to medical underwriting and coverage will not take effect until Sun Life approves your application.

How much coverage can I get?
It pays a set amount for each covered injury and treatment you receive. Some common injuries include broken bones and fractures, dislocations, ruptured discs and torn ligaments.
How can I use the money?
Because it is paid directly to you, you can use it any way you choose – including monthly bills, copays, deductibles or other expenses the medical plan doesn’t cover.

Short-Term Disability (STD) replaces a portion of your income to help make ends meet if you are totally disabled due to a covered accident or covered sickness that happens outside of work. Have you thought about what you would do if you were unable to work? How would you cover the cost of your daily living expenses? Colonial Life’s STD Insurance provides a monthly benefit to replace lost income in the event of a covered accident or illness. This coverage helps you to maintain your lifestyle.
Employees will receive a benefit of 60% of their monthly income up to $6,500. Elimination period of 7 days with a 6-month duration.

Building a strong financial foundation for your future is an ongoing process. Lift Brands’ 401(k) Plan, provided by Principal, is designed to help you save for retirement by offering you tax
advantages as well as the convenience of payroll deductions.
You can choose how much you want to save and how to invest your money. Keep in mind these key investment concepts as you make your decisions:

Eligibility and Enrollment
To be eligible to participate in the 401(k) plan you must be 21 years of age. Effective 30 days after your date of hire.
Investment Options
There are a variety of diverse mutual fund investment options offered within the plan, as well Target Date Mutual Funds aligned with participant’s date of retirement. Investment options in the
plan are routinely reviewed, and changes may be made periodically. If you would like to change your investments you can call 1-800-547-7754 or log into your account.
Employee Contributions
Employees may choose to contribute from 1% to 80% (in whole percentages) of their eligible earnings into the plan on a pre-tax or ROTH (after tax) basis into their 401(k) account. Deferrals
cannot exceed the maximum allowable dollar amount per year ($24,500 for 2026) or 80% of total compensation, whichever is less. This level is set by the IRS and is subject to change. You are always 100% vested in the contributions YOU choose to defer, you cannot forfeit these contributions.
Employee Catch-Up Contributions are also allowed for Participants age 50 or older ($8,000 for 2026).
You may change or stop making salary deferral contributions at any time. Changes will be implemented as soon as administratively feasible.
Employer Contributions
Lift Brands currently makes an employer match contribution for those who participate in the 401(k) plan. Lift Brands matches 50% on the first 5% of your salary contributed. In other words, if you elect a deferral rate of at least 5%, the company will contribute 2.5% of your salary into your account. These contributions are subject to a 3-year graded vesting schedule listed below.
Example: Employee with an annual salary of $50,000 defers 5% of annual salary.
- 5% employee deferral rate equals $2,500 annually.
- Employer matches 50% of the first 5%.
- The employee gets the full match, which will equal to $1,250 annually.
The employee will have $3,750 put into their 401(k) plan between their employee contribution and the matching employer contribution.
The vesting schedule is:
1 Year: 33%
2 Years: 66%
3 Years: 100%

In addition to the quarterly statements, you may check the value of your account, transfer between investments, change your investment allocation for future contributions, view detailed
investment performance information, request loans or in-service withdrawals, etc. by either accessing your account online at www.principal.com or calling customer service at 1.800.547.7754.
