Eligibility

ELIGIBILITY

To be eligible for benefits, you must be a full-time employee working at least 30 hours per week. Coverage for new hire employees begin on the first of the month following the date of hire.


ELIGIBILE DEPENDENTS

You may enroll eligible dependents which include:

  • Legally married spouse or domestic partner based on State regulations
  • Child(ren) up to age 26, regardless of student, marital, or employment status. This includes natural children, stepchildren, adopted (or placed for adoption), and children for whom you are legal guardian.

Medical & Rx

HukariAscendent offers three medical plan options through Gravie which utilizes Aetna's network:

  1. Comfort 5000 Plan – traditional copay-based plan with no out-of-pocket costs for non-hospital related services​
  2. HSA 3500 Plan – HDHP that is HSA-qualified​
  3. HSA 5000 Plan – HDHP that is HSA-qualified​


Gravie partners with Magellan Rx Management to provide pharmacy benefits. Reference Magellan's retail pharmacy listing below to find a participating pharmacy near you.


How do I access my Medical ID card?

  1. Login to Gravie's member portal link below (access will be available as of January 1, 2023)
  2. Select 'Download an ID Card'


How do I locate Medical Providers?

  1. Follow the link below to search the provider directory within Aetna's Signature Administrators network
  2. Enter location (zip, city or state)
  3. Set distance from home then click 'Search'
  4. Specify type of provider/facility


How do I search for prescription drugs?

  • Access the Magellan Standard Formulary and search for prescriptions with the link below
  • Request a 90-day supply with Magellan's online form link below

Health Savings Account

A qualified high deductible health plan with a Health Savings Account (HSA) is an alternative to traditional health insurance plans. The HSA allows you to set aside pre-tax dollars to pay for qualified health care expenses. You must be covered by a Qualified High Deductible Health Plan (QHDHP) to be eligible to contribute to an HSA.


Your HSA is your money. If you change jobs or retire, your HSA balance goes with you. The funds continue to roll over each year until the funds are used.


Keep in mind, HSA elections must be summited each year.

Group Life and AD&D Insurance

Life insurance can provide your dependents with a lifetime of financial security and upon your death, can be used to pay off your debts - such as credit cards and your mortgage or other expenses that could burden your family.


That's why HukariAscendent provides Group Life and Accidental Death & Dismemberment (AD&D) insurance to full-time employees at no cost to you. Be sure to keep your beneficiary designation up to date.


You also have the option to purchase additional voluntary life insurance for yourself, spouse, and dependents. Employees are responsible for premiums.


Disability Insurance

Becoming disabled is often and unexpected and a burdensome experience, and it can happen to anyone. Disability insurance can help protect your income in the event of a disability by providing you a benefit for injuries and sickness which are not work related.


Full-time employees have the option to purchase voluntary short-term disability and voluntary long-term disability through Mutual of Omaha. Employees are responsible for premiums.


Online Evidence of Insurability (EOI) Form

Those who elect more than the guaranteed issue amount(s) for voluntary life insurance and/or long-term disability will be required to submit an EOI form. Access the online EOI form link below. Once the EOI is approved by the insurance carrier, coverage will be effective the first of the month following approval.

Mutual of Omaha offers a Travel Assistance program that helps with your travels over 100 miles away from home or outside the country at no cost to you.


Will Prep Services

Mutual of Omaha has partnered with Epoq, Inc. to offer members online will prep tools at no cost to you. Complete a basic will or other documents to protect your family.

Hearing Discount
