Benefits Overview for 2023

Medical Mutual: will continue as your Medical carrier with the below changes.

Plan 1:

  • $600/$1,200 In Network Deductible
  • $1,200/$2,400 Out of Network Deductible
  •  $1,800/$3,600 In Network OOP Maximum

Plan 2: No changes

 

MetLife: will continue as your Dental, Vision, Life and Disability carrier.

 

First Stop Health: will continue as your telemedicine virtual health carrier.

 

EASE@Work: will continue to provide your employee assistance program.

Enroll & Qualifying Event Changes

Full-Time Employees

You are eligible for benefits if you are full-time working a minimum of 30 hours per week. Your coverage will be effective the 1st of the month following date of hire.

 

EMPLOYEE ELIGIBILITY: If you do not enroll during the Open Enrollment period, you must wait until the next Open Enrollment period unless you experience a Qualifying Life Event. If you experience a Qualifying Life Event (i.e., birth or adoption of a child, marriage, divorce, loss of other coverage, etc.) you must enroll within 30 days of the Qualifying Event.

 

DEPENDENT ELIGIBILITY: You may also enroll eligible dependents for benefits coverage. A ‘dependent’ is defined as the legal spouse and/or ‘dependent child(ren)’ of the plan participant or the spouse.

  • Your Dependent Children: Children are eligible up to the end of the month of 26th birthday regardless of marital or student status.

 

What is a Qualifying Life Event?

• Marriage

• Divorce

• Birth/Adoption of a child

• Death of a spouse or other enrolled dependent

• Change in spouse’s benefits or employment status

• A dependent becomes eligible for Medicare or Medicaid

 

If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.

Enrollment with EASE

All employees will have access to our online benefits enrollment platform 24/7 where you can enroll, select or change your benefits online during the annual open enrollment period, new hire orientation, and for qualifying events.

ENROLLMENT DIRECTIONS


  1. You will receive an Email with a link to Ease. Click the blue “login” button IN the email message to be directed to the enrollment portal.
  2. Enter your User ID and password
  3. If you do know your User ID and Password, please contact Lindsay Pschirer
  4. Follow instructions and enroll in your benefits.


Make sure to sign your forms, save your elections and print your confirmation statement!

HEALTH

Medical

Medical Mutual is your medical benefit carrier.


Lake Metro Parks offers 2 plan options through Medical Mutual.


Plan 1:

  • $600/$1,200 In Network Deductible
  • $1,200/$2,400 Out of Network Deductible
  •  $1,800/$3,600 In Network OOP Maximum

Plan 2:

  • $1,250/$2,500 In Network Deductible
  • $3,300/$6,600 In Network OOP Maximum


When you utilize providers that are in MMO’s network you receive higher benefits. Employees who seek care from a provider that is not in MMO’s network, will be responsible for higher costs.

Dental

MetLife is your dental benefit carrier.


You may go to any dentist. However, those who belong to the PDP Plus network will be most cost effective.



What's covered?


Preventive:

Routine exams, cleanings, x-rays, sealants, and fluoride treatments are covered 100% in and out of network


Basic:

Periodontics, endodontics, oral surgery, filings, prosthetic maintenance and full mouth x-rays are covered at 80% in network


Major:

Prosthodontics, crowns, inlays/onlays, dentures, implants and bridges are covered at 50% in network


Orthodontia:

Covered at 50% in network for dependents up to age 26

Basic & Voluntary Life

Life InsuranceMetLife is your Basic and Voluntary Life carrier.


Life Insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. Accidental Death and Dismemberment (AD&D) insurance is equal to your Life benefit in the event your death is a result of an accident and may also pay benefits for certain injuries sustained. MetLife is your Basic and Voluntary Life carrier.


It is important to name your beneficiary! A Beneficiary is the person you designate to receive your life insurance benefits in the event of your death. It is important that your beneficiary designation is clear so there is no question as to your intentions. 


Your Basic Life Benefit

  • 100% company paid benefit, no cost to you.
  • Flat $21,000 benefit 

 

Your Voluntary Life Benefit

  • Up to $150,000 or 5x annual salary
  • Available to active full-time employees (40 hours/week)
  • 100% employee paid benefit.
  • Spouse and dependent coverage available.

  • You will become ill when traveling

How does it work?

  • Call (888) 691-7867 or log in to fshealth.com or download the mobile app to request your doctor visit
  • Set up your account now so its ready when you need it!
  • Complete patient registration for medical history, pharmacy preference, primary care physician contact information and insurance information.
  • Each time you visit you will be asked brief medical questions. If appropriate you will be connected to a doctor using secure live audio and video.
  • You and your doctor will discuss your medical issue and if appropriate your doctor may write a prescription.
