Open Enrollment is typically your opportunity to evaluate your current benefit elections and make changes for the new plan year; however, we have some big changes coming that will take effect July 1. For that reason, we are going to extend your current benefit elections until then, with the exception of the Identity Theft Protection plan (see the Changes This Year section). All other benefits will automatically renew, and you will be given a full open enrollment for a July 1 effective date.
Vision benefits: We are increasing your frame allowance to $200/year.
Identity Theft Protection: We are moving these benefits to NortonLifeLock. If you are currently enrolled with Aura Identity Guard, that policy will be cancelled as of April 1st, so you will need to complete a new application, if you wish to participate in the plan with NortonLifeLock. See your benefits packet for details.
Your Cost Per Paycheck: The only benefit that incurred an increase is vision. The Casino is absorbing the increase for you, so you will only see an increase to your payroll deductions, if you have your family members enrolled.
As a full-time Team Member of the Kiowa Casino, you are eligible for the comprehensive benefits available to you and your family.

Enrollment Instructions
For currently enrolled Team Members, your benefits packet includes your personal Benefits Statement, which reflects your current benefit elections and payroll deductions. Please review for accuracy and report any discrepancies to HR. Note: Your benefits statement might not reflect any recent election changes.
For newly benefit eligible Team Members, your packet includes a benefits application. Please complete this form and return to HR no later than your 60th day of employment.
For anyone interested in enrolling in the NortonLifeLock Identity Theft Protection plan, you must complete their application included in your benefits packet.
Your NortonLifeLock application must be submitted to HR by March 13th, if you wish to participate in the identity theft protection plan. No other paperwork is required.

Keep in mind we are switching to the Native Blue plan July 1, so these plans will be cancelled as of 6/30/2023.

If you choose to enroll in the HSA Qualified High Deductible Health Plan (HDHP) with BCBS, you can elect to participate, and specify your annual contribution, on your Application/Change form.

As your employer, we automatically provide a certain level of coverage for you and give you the opportunity to purchase additional coverage for you and your dependents.
Newly eligible Team Members may purchase up to $100,000 Voluntary Life Insurance without providing Evidence of Insurability (EOI). EOI is required for any election that exceeds $100,000 and for members that elect this benefit after their initial enrollment period.
In addition, Team Members that elect coverage for themselves, can elect coverage for their spouse and dependent children. Newly eligible Team Members may purchase up to $50,000 Spousal Voluntary Life insurance without providing EOI. EOI is required for any election that exceeds $50,000 and for late enrollees. Children can be insured for $10,000.
