Keystone Profiles is pleased to offer an extensive selection of benefits designed with your health and financial wellbeing in mind. We review our benefit program each year to ensure our offerings provide the best combination of benefit coverage, network access and affordability.
Eligibility
Eligibility
If you are a regular, active, full-time employee and work a minimum of 40 hours per week, you are eligible for the company benefits. You are eligible to participate on the date of hire.
In addition, the following family members are eligible for certain benefits as well:
- Your legal spouse. This does not include a common law spouse or an ex-spouse.
- Your (or your spouse's) biological child, step child, a child for whom legal guardianship has been awarded, and who are less than 26 years of age.
- Unmarried dependent child who is physically or mentally disabled and is dependent on you.
Qualifying Events
You can change your benefit election during the year if you have a qualifying life event. Qualifying life events include, but are not limited to:
- Marriage, divorce or legal separation
- Birth or adoption of a child
- Change in child’s dependent status
- Death of a spouse, child or other qualified dependent
- Change in employment status or a change in coverage under another employer-sponsored plan
If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.
Medical
Keystone Profiles offers a medical plan option for full-time employees through UPMC Health Plan. It provides high-quality, affordable medical care, including preventive care, doctor's visits, hospitalization, emergency care and prescription drug coverage. HSA
Health Savings Accounts (HSAs) are a great way to save money and budget for qualified medical expenses.
The maximum amount you can contribute to an HSA in 2025 is $4,300 for individual coverage and $8,550 for family coverage.
Additionally, if you are age 55 or older, you may make an additional “catch-up” contribution of $1,000. You may change your contribution amount at any time throughout the year as long as you don’t exceed the annual maximum.
In order to contribute to an HSA, you must not:
- Be claimed as a dependent on someone else’s tax return
- Have other health plan coverage that provides benefits covered under the HDHP Plan (for example, a Health Care Flexible Spending Account, a separate prescription drug plan, or a spouse’s non-HDHP plan)
- Have a spouse with a Health Care Flexible Spending Account that could reimburse your medical expenses
- Be enrolled in a government health plan, such as Medicare or Medicaid
- You must re-enroll in your HSA each year during open enrollment to participate in the plan

Your dental benefits are through Guardian Life Insurance Company. Please refer to the benefits summary below for more information.
Vision

Basic Life/AD&D
Keystone Profiles provides eligible employees with $25,000 in group life and accidental death and dismemberment (AD&D) insurance administered by Guardian.
Keystone Profiles pays for the full cost of this benefit—meaning you are not responsible for paying any monthly premiums.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Short-Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
- Benefits begin after a 8-day waiting period. The benefit pays 60% of your income to a maximum of $400 per week.
