How To Enroll

Enrollment Forms
  • If you'd like to change your existing coverage or make new elections, you must do so by November 30.
  • Your CURRENT medical, dental and vision elections will carry forward to 2021-2022 if you don't make a new election.
  • Anyone enrolled in the medical coverage will automatically be enrolled in the Nationwide/Gilsbar supplemental plan.
  • You will be receiving cards for this new Nationwide/Gilsbar plan please REVIEW the video in Benefits Overview Section on what it looks like and how it works.
