Plan Year Dates

Jones County School System’s benefit plan year begins on January 1st, 2025, and ends on December 31st, 2025. This timeframe defines the period during which your benefits are effective.

Please refer to this timeframe when making benefit elections, submitting claims, or requesting changes to your coverage.

This section helps you understand who qualifies for benefits and important timelines for enrolling or making changes. Check your plan details below to confirm specific rules that apply to you and your family.

Employee Eligibility

Active full-time employees of Jones County School System are eligible for benefits. For any eligibility questions, please refer to the New Hire Information tab or contact your HR or Benefits Team.

Dependent Eligibility

Eligible dependents typically include:

• Your legal spouse

• Your children up to age 26 (natural, adopted, or stepchildren)

• Children over age 26 who are unable to support themselves due to a physical or mental disability

⚠️ Please Note: Enrolling someone who does not qualify as a dependent is considered insurance fraud and may result in penalties.

📄 Dependent Verification:

Jones County School System may require verification of dependent eligibility. This could include submitting documents such as a marriage certificate, birth certificate, or legal guardianship papers. Until these documents are submitted and approved, dependent benefits may be placed on hold or pending.

New Hire Info

You’ll have 30 days from your hire date to enroll in your benefits.

After you make your elections, your benefits will become effective on 1st of the month following 30 days from date of hire.

For example:

If you're hired on June 18, your benefits would begin on August 1

Qualifying Life Events

When Can I Change My Coverage?

Your benefit elections typically remain in place for the entire plan year—unless you experience a Qualifying Life Event (QLE) that allows you to make changes.

Common QLEs include:

• 🧑‍🤝‍🧑 Changes in marital status

Marriage, divorce, legal separation, annulment, or death of a spouse.

• 👶 Change in dependent status

Birth, adoption, placement of a foster child, legal guardianship, or death of a covered child.

• 👔 Employment changes

Gain or loss of employment for you or your spouse, changes in work schedule (full-time/part-time), or return from unpaid leave.

• 📍 Change in residence

A move that affects benefit eligibility for you, your spouse, or dependents.

• 🩺 Eligibility for other coverage

Gaining or losing coverage through Medicare, Medicaid, CHIP, PeachCare, or a spouse’s employer plan.

⚠️ Important:

You must report your Qualifying Life Event within 30 or days of the event. Failure to act within this window may result in delayed or denied changes, and you will have to wait until the next Open Enrollment period.

📲 How to Declare a QLE

  • Medical coverage (State Health Benefit Plan):

You must contact State Health Benefit Plan Member Services directly at (800) 610-1863 or visit Sign In | OBASHPB to report your medical QLE and make changes.

  • Non-medical benefits (dental, vision, life, disability, FSA, etc.):

Contact the NFP Call Center at (800) 994-7429 or email NFPseCustomerService@nfp.com to initiate changes.

We recommend keeping records of any confirmations or receipts you receive during the change process.

💡 Note: A newborn is typically covered for the first 30 days after birth. To continue coverage beyond this period, you must enroll the child within the QLE window. Otherwise, coverage may be terminated retroactively to the date of birth.

As an eligible employee of Jones County Schools, you have access to affordable healthcare through the State Health Benefit Plan (SHBP), which offers a variety of options including HMO, HRA, and High-Deductible Health Plans.

Members are encouraged to visit the SHBP website regularly for updates, tools, and resources. Please note: SHBP Member Services has posted “Invalid/No Social Security Number (SSN) FAQs” under the Member Rates & Information tab. If an invalid SSN is provided for any dependent, their coverage may be terminated if a valid SSN is not submitted.

An FSA allows you to set aside pre-tax dollars to pay for eligible medical, dental, vision, and/or dependent care expenses. These contributions reduce your taxable income and can be used throughout the year to cover qualified out-of-pocket costs.

🧾 Participation Reminders:

You are required to re-enroll in the FSA plans each year to maintain coverage.

Flexible Spending Accounts (FSAs) allow you to reduce your taxable income by setting aside pre-tax dollars from each paycheck to pay for eligible out-of-pocket health care and dependent care expenses for yourself, your spouse, and your dependent children.

Voluntary Life Insurance allows you to purchase additional coverage for yourself, your spouse, and your eligible dependents through payroll deduction. This optional benefit offers added financial protection and peace of mind.

💼 Eligibility

Available to all eligible employees of Jones County Schools

🧍 Employee Coverage

  • Elect in $10,000 increments
  • Up to a maximum of $300,000
  • New Hires: May elect up to $150,000 with no medical questions (Guaranteed Issue)
  • Current Employees: May increase by 2 increments (up to $20,000) during open enrollment without medical questions
  • Evidence of Insurability (EOI) is required:

- For any amount elected above the Guaranteed Issue

- If applying outside of your initial eligibility period

💍 Spouse Coverage

  • Elect in $5,000 increments
  • Guaranteed Issue: $25,000 (not to exceed 100% of employee amount)
  • EOI required:

- For any amount above the Guaranteed Issue

- If coverage is elected after the initial eligibility period

  • Spouse rates are based on the employee’s age

👶 Child(ren) Coverage

  • Elect in $2,000 increments, up to $10,000 (cannot exceed 100% of employee amount)
  • Covers dependent children from birth through age 26
  • No EOI required

📄 EOI Process

If you or your spouse are required to submit an Evidence of Insurability (EOI) form, it is your responsibility to complete and send the form to the provider at the address listed on the form. Coverage amounts above Guaranteed Issue will not be approved without EOI.

📉 Age Reduction Schedule

Coverage reduces to 50% of the original amount at age 70

🔁 Portability

Coverage may be continued at group rates if your employment ends

🔄 Conversion

Coverage may be converted to an individual permanent life policy without medical questions when your group coverage ends

📝 Beneficiary Designation

You must name a beneficiary when enrolling and may update your designation at any time.

• Employee: Up to $300,000

• Spouse: Up to $100,000 (not to exceed employee’s amount)

• Children:

– $25,000; $10,000 Child Term Life Rider available at a flat weekly rate

– Coverage lasts until the child’s 26th birthday

– May be converted to a Whole Life policy at that time

Short- and Long-Term Disability insurance provides income protection if you’re temporarily or longer-term unable to work due to illness, injury, or accident. This benefit helps replace a portion of your income so you can focus on recovery, not financial strain.

💡 Key Features

  • Income Replacement: Up to 66.67% of monthly earnings, up to $8,000 per month
  • Elimination Period (waiting period before benefits begin): Choice of 0, 3, 14, 30, 60, 90, or 180 days depending on accident or other disability
  • Benefit Duration: Up to Social Security Normal Retirement Age (SSNRA) or 5 years for certain plans
  • Pre-Existing Condition Clause: Disabilities related to treatment, consultation, or medication in the 6 months prior to coverage may not be covered during the first 12 months
  • Limitations: Outpatient mental/nervous illness (24-month lifetime payout), intentional self-inflicted injuries, disability due to war, and substance abuse (24-month limitation)
  • Enrollment: Two opportunities—within 30 days of hire or during annual open enrollment; no health questions for new hires, late entrants subject to pre-existing condition rules
  • Sick Leave Offset: Not reduced by accrued sick leave
