It is important to the City of Inver Grove Heights to help you and your family reach and maintain good health. The City of Inver Grove Heights is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. The City reviews the benefit programs each year to ensure they consider the best combination of benefit coverage, network access, and affordability for all employees.


Eligibility

Regular employees working at least 20 hours per week are eligible to participate in benefits offered by the City of Inver Grove Heights. When you enroll in the benefits program, you may also cover your eligible benefits. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.







ENROLLMENT FORMS

The Benefit choices you made during your initial enrollment or annual open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

There are three medical plan options with eligibility for employee, spouse, and dependent coverage.


Traditional Copay Plan (Not available to new enrollees)

Deductible: $250 single | $750 family

Office visit: $25 copay

Coinsurance: 20% after deductible


High Deductible HRA VEBA Plan

Deductible: $2,000 single | $4,000 family

Office visit: 0% after deductible

Coinsurance: 0% after deductible


High Deductible HSA Plan

Deductible: $2,800 single | $5,600 family

Office visit: 0% after deductible

Coinsurance: 0% after deductible

Providers are part of the HealthPartners network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher.

LIFE & DISABILITY

You can't always predict - or control - your life. But, you can prepare for it. Protecting the financial interest of your loved ones in the event of your death or serious injury can be invaluable. The City of Inver Grove Heights provides you with Basic & Voluntary Life/AD&D coverage through Minnesota Life, as well as Short- and Long-Term Disability through Colonial Life and Madison National Life respectively.


Basic Life/AD&D

Employees working 20 or more hours per week are eligible for one times annual earnings up to $200,000 of coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.


Voluntary Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage through Colonial Life pays 66 2/3% of your income to a maximum of $5,000 per week.


Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability offered through Madison National Life and provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a 90 day elimination period are are equal to 66.67% of covered monthly earnings up to $5,000 for full-time employees.

Flexible Spending Accounts

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


Contributions

The election you made during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


Health Care FSA

  • Up to $2,850 annually
  • All benefit eligible can participate, unless you or your spouse are contributing to an HSA.
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses


Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns


How the Plan Works

You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until March 31st annually to file your claims. Any funds after March 31 unclaimed will be forfeited.

Health Savings Account

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.


To be eligible for an HSA, you must be enrolled in a High Deductible Health Plan (HDHP).


You may contribute as follows:

$3,650 for a single contract

$7,300 for a two person or family

Catch up - if you are age 55 or older, you may make an additional contribution of $1,000

HRA/VEBA

A voluntary Employee Beneficiary Association (VEBA) account allows employers to contribute money to a trust on behalf of their employees. VEBAs can be used to help pay for current or future eligible medical expenses.

Employee Assistance Program (EAP)

The Employee Assistance Program supports both personal and professional well-being through acute, solutions focused counseling. EAP services are provided through Sand Creek and offers help with relationship issues, work pressures and demands, alcohol and chemical use problems, loss and grief, and much more.
