Welcome

Your benefits are an important part of your overall compensation. Hope Community Services remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. This site provides highlights of the 2025 benefit plans include:

  • New High Deductible Health Plan Option with a Health Savings Account

Enrollment Info

All FULL-TIME employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Hope Community Services. Newly hired employees' benefits will be effective the 1st of the month following 60 days from date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.

Children are eligible for benefits until the last day of the month in which the child turns 26.

Should your employment with Hope end, your coverage will end the last day of that month.

The Benefit choices you make during your open enrollment remain in effect for the entire year.

You can, however, modify your elections under certain circumstances throughout the year, called "Qualifying Events". These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event. Be prepared to show documentation for the event such as a marriage license, birth certificate, divorce decree, or letter/proof of loss or gain of other coverage.

Contact Human Resources if you have questions about qualifying events. If you benefit changes are not submitted on time, you must wait until the next Open Enrolment period.

Medical & Rx

Blue Cross and Blue Shield of Oklahoma will continue to administer our medical and prescription drug benefits.

FSA - Going Away

Flexible Savings Account will NOT be continuing in 2025.

  • Please spend remining funds that are in your account by December 31, 2024.
  • Submit reimbursement recipes before December is over.
  • No funds will rollover. Remaining funds will be forfeited.
  • This benefit is not eligible to be "cashed out".
  • You can spend unused funds at FSAStore.com (see link below).
  • Check your balance by logging in through the below link or call WEX at 1-866-451-3399

HSA

New! HSA Bank will be administering our Health Savings Account.

HSA Overview:

  • Only available when enrolled in a High Deductible Health Plan (HDHP).
  • Ineligible if:
    • you are enrolled in Medicare, Indian Heatlh Services or Military (Tricare) Coverage.
    • your spouse is enrolled in a Flexible Spending Account (FSA).
    • you can be claimed as a dependent on someone's tax return.
  • You can be enrolled in a HDHP/HSA and your spouse can be enrolled in a PPO plan with copays.
  • A bank account in your name. If you leave employment this account stays with you.
  • Contributions are pre-tax dollars automatically put in your account (employer and employee payroll deduction).
  • Funds are available as they are contributed per pay period.
  • Funds are used to pay for qualified medical, dental, vision and some over the counter expenses tax free.
  • Unused funds rollover from year to year and accumulate tax free.
  • At age 65 you can use the funds in this account for non-eligible expenses, but you will have to pay income tax.

Basic Life & AD&D

Hope Community Services provides all full-time with the following:

Employee - $50,000

Spouse - $5,000

Child(ren) - $2,000

Who's Your Beneficiary? Everyone needs to make sure your beneficiary is up to date.

Don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

Life Policy Conversion is available if you want to keep this coverage after you leave employment. You may apply to convert this policy to an individual whole life policy by completing the below application and following the directions on the form within 30 days of leaving employment.

Voluntary Life and AD&D

Employees may purchase additional life and AD&D insurance. Per pay period rate is based on your age and benefit amount selected. Please see the below "Voluntary Life Rate Grid" document for plan cost.

Employee - $10,000 increments up to $300,000. Guarantee Issue for new hires - $100,000

Spouse - $5,000 increments up to $150,000. Guarantee Issue - $50,000

Child(ren) - $10,000 for ages 15 days to 26 years (30 years for full-time student).

Evidence of Insurability (EOI) Form may be required if you are adding new or addental coverage (over $10K for employee and $5K for spouse) at open enrollment. BCBS will either approve or deny this additional coverage based on your EOI form.

Disability

Disability insurance helps protect your income in the event you are unable to work due to a disability related to illness or injury. Per pay period cost is based on your age and weekly or monthly salary.  

Voluntary Long-Term Disability

After 180 days of disability the plan pays you 60% of your monthly pre-disability income, up to $6,000 a month, until you reach Retirement Age or as long as you are no longer disabled, whichever is sooner.

Pre-existing limitation (12/12) - if you have received medical treatment, consultation, care or services including diagnostic measures and/or prescription medication in the12 months prior to your effective date of coverage that results in a distality during the first 12 months of coverage, would not be covered.

Voluntary Short-Term Disability

After 14 days of disability the plan pays you 60% of your weekly pre-disability income, up to $1,500 a week, for up to 24 weeks.

Pre-existing limitation (3/6) - if you have received medical treatment, consultation, care or services including diagnostic measures and/or prescription medication in the 3 months prior to your effective date of coverage that results in a distality during the first 6 months of coverage, would not be covered.
