November 28th - December 6th
Open Enrollment is your annual opportunity to update and learn about the benefits provided through your employer. This is a good opportunity to review your current coverage and choose the best options for you and your family. All benefit changes during open enrollment will become effective January 1, 2024.
You must complete the Consolidated Enrollment Form that was sent to you even if you are making no changes. Remember, payroll deductions to an FSA must be reelected each year, there is a separate enrollment form for that benefit under the FSA section of this site.
Benefit changes outside of Open Enrollment require a Qualifying Life Event and/or Loss of Eligibility.
You may not change your benefits throughout the year unless you experience a Qualifying Life Event. Qualifying Events may include marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you must notify HR within 30 days of the event. Please remember you only have 30 days from the event to make changes to your benefit elections.

Disability insurance helps protect your income in the event you are unable to work due to a disability related to illness or injury. Per pay period cost is based on your age and weekly or monthly salary.
Voluntary Long-Term Disability - New!
After 180 days of disability the plan pays you 60% of your monthly pre-disability income, up to $6,000 a month, until you reach Retirement Age or as long as you are no longer disabled, whichever is sooner.
Please note: We are newly offering the Voluntary Long-Term Disability coverage this year. If the minimum participation is not met for the plan, then the plan will not be offered.
Voluntary Short-Term Disability
After 14 days of disability the plan pays you 60% of your weekly pre-disability income, up to $1,500 a week, for up to 24 weeks.

Basic Life & AD&D
Hope Community Services provides all full-time with the following:
Employee - $50,000
Spouse - $5,000
Child(ren) - $2,000
Who's Your Beneficiary? Everyone needs to make sure your beneficiary is up to date.
Don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Life Policy Conversion is available if you want to keep this coverage after you leave employment. You may apply to convert this policy to an individual whole life policy by completing the below application and following the directions on the form within 30 days of leaving employment.
Voluntary Life and AD&D
Employees may purchase additional life and AD&D insurance. Per pay period rate is based on your age and benefit amount selected. Please see the below "Voluntary Life Rate Grid" document for plan cost.
Employee - $10,000 increments up to $300,000. Guarantee Issue for new hires - $100,000
Spouse - $5,000 increments up to $150,000. Guarantee Issue - $50,000
Child(ren) - $10,000 for ages 15 days to 26 years (30 years for full-time student).
Evidence of Insurability (EOI) Form may be required if you are adding new or addental coverage (over $10K for employee and $5K for spouse) at open enrollment. BCBS will either approve or deny this additional coverage based on your EOI form.
