Open enrollment is your opportunity to learn about your health care benefits.
It is a time to add or change any coverages for you and your family.

UPMC Medical Plan - Gold EPO $2,500 (NEP3T)
UPMC Vision Care Premier Plus $0 Copay
UPMC Dental Advantage Premium 100/80/50/$0/$1,500/No Ortho
Nationwide GAP Coverage - Gilsbar Plan (out of pocket FIRST is $1,000 individual / $2,000 family)

How To Enroll
- If you'd like to change your existing coverage or make new elections, you must do so by November 11.
- If you need to make changes or add additional coverage(s) please download and complete the UPMC Employee Benefit Election and Change form found on the left.
- Note: NO ACTION is required if there are no changes needed - Your current medical, dental and/or vision elections will carry forward to the new plan year.
