Open Enrollment is from November 17th through November 24th. This is the time of year where you can elect new plans with Howard & Howard. You may switch between plans and make changes without being required to have a qualifying life event. Please also keep in mind the following when making your elections for 2026:

All FULL-TIME employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Howard & Howard. Newly hired employees will be eligible the 1st of the month following date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Children are eligible for benefits as follows:
Should your employment with Howard & Howard end, your coverage will end as of the date of termination.
ENROLLMENT INSTRUCTIONS:
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Blue Cross Blue Shield of MI (BCBSM) will continue to be your medical and prescription carrier. You have the choice between two plans for the 2025 plan year. The two plan options include a PPO plan, a PPO HSA plan. Both medical plans offered are Preferred Provider Organizations (PPO); this means that both in and out of network coverage is provided, you are not required to select a primary care physician, and you are not required to obtain a referral to see a specialist.

If you are already enrolled in one of the Howard & Howard BCBSM medical plans today and have not already done so, please be sure to register through the BCBSM member portal where you can access important information pertaining to your medical plans with BCBSM including participating providers, cost estimates for services, explanation of benefits/claims, and accumulation amounts for deductibles and out-of-pocket costs. If you are not currently covered, you will be able to register in the BCBSM portal as of your effective date of coverage. Information on how to register can be found here.

Look up your prescriptions by viewing the BCBSM Drug List. Use the Control F feature to type in the name of your prescriptions and determine which level the drug is and whether a Prior Authorization (PA) or Quantity Limits (QL) apply. You can also determine whether there may be lower cost alternatives for your prescriptions.

MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. Helpful Rx Cost Saving Tools & Tips!
PAYER MATRIX - NEW FOR 2025 (only available on the PPO medical plan)
Eligible members will receive direct communication from PillarRx and Payer Matrix through a co-branded letter and phone calls to inform them about the program. This initiative offers discounts on approved medications. Payer Matrix will gather information from members, including income details, to determine their eligibility for the program.
If a member is denied a discount on their medication, they will receive it through the standard PillarRx program and will pay their usual copay or coinsurance.
THE MEMBER WILL BE RESPONSIBLE TO PAY 100% OF THE COST OF THE MEDICATION AT THE PHARMACY, IF THE MEMBER DOES NOT RESPOND.
Note: The pharmacy will also receive a notification that the member must contact PillarRx. There is still opportunity to receive the discount once you contact PillarRx.
How can members contact Payer Matrix?

PILLAR Rx You may be contacted by BCBSM via phone or mail to sign up for PillarRx, it's important you don't ignore these calls or letters. The team will only contact you when a drug you're taking is now under a 'co-pay assistance program' called PillarRx. If you don't register for PillarRx, which is free to do, you will be asked to pay a much higher copay than you are used to. Click the "Pillar Rx" button below to learn more.
GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).
ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.

If you do seek care out of network, you will experience higher cost sharing in addition to possible balance billing. You have the opportunity to contribute to a variety of savings and/or spending accounts on a pre-tax basis which lowers your taxable income. View the information below for important details and guidelines for the various accounts offered by Howard & Howard.
The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. The PPO 2000 Medical Plan is qualified for an HSA. By contributing to an HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
HSA IRS Maximums are as follows and are on a calendar year basis. The maximums indicated below are a combined maximum for participant and any employer provided funds to the HSA. If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.
If you are only covered by the HSA PPO plan for part of the 2025 calendar year, your maximum allowable contribution is prorated based on your effective date of coverage on the HSA PPO plan.
If you have an HSA with another bank or custodian, you can transfer your funds to the Howard & Howard HSA with Health Equity by completing this form.

The Flexible Spending Account (FSA) allows you to set aside pre-tax dollars to pay for eligible medical and/or dental and vision expenses. The nice part about an FSA is you have access to the full annualized amount on day one!
The election you make cannot be changed throughout the year unless the change aligns with the qualifying event. For example, if you get married, you can increase your contribution, however, you cannot decrease or end your FSA for this type of life event.

The Dependent Care Reimbursement Account (DCRA) allows you to set aside pre-tax dollars to pay for eligible dependent care expenses. Qualifying expenses include daycare, nursery & preschool tuition, summer day camps, and more!
The election you make cannot be changed throughout the year unless the change aligns with the qualifying event. For example, you can increase your contribution if you add another member to your family through birth or adoption, however, you cannot decrease your contribution for this type of life event.

Howard & Howard's dental plan provides comprehensive coverage to help you, and your family maintain good dental health. The dental plan includes both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
Dental is now a standalone benefit. You can elect Dental without electing Medical coverage.

Howard & Howard offers vision coverage through BCBSM (who partners with VSP) to help pay for eye exams, prescription glasses and contact lenses. BCBSM also provides discounts for Hearing Care and Lasik.

Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Open Enrollment Increase Option: During the annual open enrollment period, you will be able to increase your election up to the Guarantee Issue without proof of good heath as long as you weren’t previously declined or withdrawn.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
