BASIC LIFE/AD&D

Life insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. AD&D insurance is equal to your Life benefit in the event of your death being a result of an accident and may also pay benefits for certain injuries sustained. Basic Life Insurance and AD&D are provided by Mutual of Omaha Insurance. This benefit is employer paid.


Coverage Amount: $50,000

AD&D Amount: Amount equal to your Life benefit

VOLUNTARY LIFE/AD&D

Mutual of Ohama

Voluntary Life and Accidental Death & Dismemberment plans are offered for you to elect coverage. Employees must be enrolled to elect coverage for dependents.


Coverage


Employee: $10,000 minimum and a maximum of no more than 5 times your annual salary.

Spouse: $5,000 minimum and a maximum of 100% of employees benefits up to $100,000.

Children: $5,000 minimum and a maximum of 100% of employees benefits up to $10,000.

LONG TERM DISABILITY

Mutual of Omaha

Long-term disability insurance coverage allows you to maintain a portion of your income if you are disabled, and helps you return to work when you are ready. All active, full-time employees working 30 or more hours per week will be eligible. The premiums for this insurance is paid in full by the policyholder.


Your benefits are equivalent to 60% of your pre-taxed weekly earnings.

SHORT TERM DISABILITY

You must actively be working a minimum of 30 hours per week to be eligible for coverage.

The premiums for this insurance is paid in full by the policyholder. There is no upfront cost for the insurance.

Your benefits are equivalent to 60% of your pre-taxed weekly earnings.


Maximum benefit period: Up to 11 weeks

Maximum weekly benefit: $1,500

Minimum weekly benefit: $15
