ELIGIBILITY

As a Holiday Health Care employee, you have access to comprehensive medical coverage to protect you and your family from catastrophic medical costs. The following is a summary of the information on the medical plans offered. Take the time to understand how these plans work, the coverage each provides, and how to use them to best meet the needs of you and your family.


Eligibility Criteria:

• You must work a minimum of 30 hours a week in order to be eligible for Full Time Benefits.

• You must work a minimum of 20 hours a week in order to eligible for Part Time Benefits.

• You become eligible for benefits after 60 days of employment. Benefits would be effective the first of the month following 60 days of employment.


Eligible Dependents:

• Legal Spouse

• Children up to the age of 26, regardless of student status or marital status. This includes biological children, stepchildren, and legally adopted or soon to be adopted children who are your dependents or for whom you are required to provide health care coverage under a Qualified Medical Child Support Order.

Holiday Health Care offers 2 medical plans through Anthem:

  • PPO Plan
  • High Deductible Health Plan (HDHP) with Health Savings Account


Included with your medical plan is The Difference Card (see next section for full breakdown). The Difference Card is designed to alleviate some out of pocket costs for you and your family.

What is The Difference Card?

The Difference Card is a benefit account funded entirely by your employer. It’s designed to help you save money on out-of-pocket medical expenses that you may incur under your health insurance plan. Your Difference Card can be used to help pay for the following expenses:

  • Office visit copays
  • Emergency room copays
  • Deductible and coinsurance expenses


A MasterCard will be sent to those enrolled in the PPO Plan to use for copayments. Any service subject to deductible, including all HDHP enrollees: See below document outlining instructions for submitting reimbursement form.

Vision Coverage is provided by Sun Life through Vision Savings Plan (VSP).


FIND A VSP Provider:


You will have access to the largest national network of private-practice eye care doctors in the industry through Vision Service Plan (VSP).

There are four ways to find an in-network doctor:

  1. Visit vsp.com and select the Choice network
  2. Call VSP at 800-877-7195
  3. Download the mobile app – Benefit Tools and search for a doctor near you

LIFE INSURANCE

Sun Life

Term Life Insurance can help protect your loved ones' financial health if you are no longer there to support them.


For you: You can choose from $10,000 to $500,000 – In increments of $10,000 not to exceed 5 times your Basic Annual Earnings. No medical questions asked up to the Guaranteed Issue Amount of $150,000.


For your spouse: If you elect coverage for yourself, you can choose from $5,000 to $100,000 – In increments of $5,000. No medical questions asked up to the Guaranteed Issue amount of $50,000.


For your child(ren): If you elect coverage for yourself, you can choose $1,000, $2,000, $4,000, $5,000 or $10,000. No medical questions asked up to the Guaranteed Issue amount of $10,000.

  • The amount you select for your child(ren) cannot exceed 100% of your coverage amount.
  • Child(ren) must primarily depend on the employee for 50% or more of their support.
  • A full benefit is payable for a dependent child who is 6 months to 26. (No benefit is payable for a child from birth to 6 months).


This coverage includes Accidental Death and Dismemberment.

SHORT TERM DISABILITY

Short Term Disability protects your income when you can’t work. If you’re unable to work because of a covered disability, Short Term Disability insurance replaces a portion of your income in addition to providing other services and benefits that help you return to work. Provides you with a weekly check. After your claim is approved, you will receive a check for your benefits that helps you pay everyday expenses like your mortgage or rent, childcare, and groceries.

CRITICAL ILLNESS

Critical Illness coverage can:

  • Help Protect Your Finances from an Illness. When you, your spouse, or child is diagnosed with a covered condition, you can receive a cash benefit to help pay unexpected costs not covered by your health plan.
  • Help Cover Related Expenses. While health plans may cover direct costs associated with a critical illness, you can use your benefit to help with related expenses like lost income, childcare, travel to and from treatment, deductibles and copays.
  • Pay a Cash Benefit Directly to You.


Critical Illness insurance can be used however you want, and it pays in addition to any other coverage you may already have.

ACCIDENT INSURANCE

Accident coverage can:

  • Help your finances after a mishap. When you, your spouse, or child has a covered accident, like a fall from a bicycle that requires medical attention, you can receive cash benefits to help cover the unexpected costs.
  • Help cover related expenses. While health plans may cover direct costs associated with an accident, you can use accident benefits to help cover related expenses like lost income, childcare, deductibles and copays.
  • Pay cash benefits directly to you. Accident Insurance can be used however you want, and it pays in addition to any other coverage you may already have. Benefits are payable directly to you. And get this – there are no health questions or pre-existing conditions limitations.
