Introduction

We are pleased to present this microsite which highlights the Employee Benefits Open Enrollment materials and makes them readily available to you at the touch of your finger or the click of your mouse. We have made many exciting changes to your Employee Benefits for 2021. BCBS will continue to be our medical insurance carrier and we are making plan design changes and network changes, you will have your choice of two plans. Delta Dental will be our dental provider and just like the medical plans, you will have two plans to choose from. Vision and Life are changing carriers, however benefits are remaining the same. You will have an opportunity to buy additional Life insurance, Disability, both short term and long term and we are adding a Flexible Spending Account!   Our benefits program is designed to offer employees substantial coverage to meet both individual and family needs. This page includes only highlights. The specific terms of coverage, exclusions & limitations are contained in the plan documents and insurance certificates. All coverage and coverage costs are subject to change at any time in the future. If you have any questions about a specific service or treatment, please contact the appropriate insurance carrier.

How To Enroll

  1. Enrollment forms must be completed for BCBS Medical, Delta Dental, Ameritas Vision, BCBS Basic Life and Voluntary Life, Mutual of Omaha STD and LTD and FSA. There are a lot of forms, however all forms need to be brought up to date for your HR department.
  2. Submit the enrollment form(s) to Human Resources no later than December 15th.
