We have some important updates to share about your health and wellness benefits—please take a few minutes to review, as some changes require action from you.

What’s Changing

Over the past year, we have been reviewing our employee health benefit program to ensure you are receiving the best and most affordable health benefit program. Here’s what you need to know:

🌿 Wellness Programming

  • New Partner: We’re thrilled to welcome PeopleEQ, a nationwide wellness provider with a user-friendly app and website. Stay tuned for rollout details later this year! - This launches November 3.
  • EAP: We have partnered with CuraLinc to offer a new Employee Assistance Program, offering extra benefits to make your lives easier, effective 1/1/2026.
  • Virtual Urgent Care: Starting on 1/1/2026, we are proud to offer a new telehealth urgent care service via First Stop Health. Experience medical diagnoses and more at your fingertips!
  • New Pharmacy Discounts: We now offer discounts on high-cost pharmacy scripts via a new partnership with ImpaxRX, effective 1/1/2026.
  • Expanded Rx Offerings: RxBenefits will be adding the SaveOnSP program which lowers the financial burden of specialty medications for plan participants.

Eligibility

EMPLOYEE ELIGIBILITY

You are eligible to participate if you are full-time employee working at least 30 hours per week.

New Hires: Your coverage will be effective first of the month following date of hire.

DEPENDENT ELIGIBILITY

You may also enroll eligible dependents for benefits coverage. A ‘dependent’ is defined as the legal spouse and/or ‘dependent child(ren)’ of the plan participant or the spouse.

OPT OUT INCENTIVE

The City of Gahanna offers an 'opt out' incentive each year to union members who waive medical coverage offered by Gahanna and can demonstrate they have other coverage. You must complete the waiver form and submit to HR.

QUALIFYING EVENTS

You can modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, or loss of eligibility under another plan. If you experience a qualifying event, you must make changes to any and all benefits within 30 days of the event.

How to Enroll

Enrollment will be submitted through our Employee Self-Service Site.

  • Employees will be able to complete most of the enrollment tasks online. Paper forms will not be accepted unless indicated.
  • Only specific paper forms will still need to be completed, depending on the benefit type (see OPTIONAL ITEMS below).

  • Each time you visit you will be asked brief medical questions. If appropriate you will be connected to a doctor using secure live audio and video. You and your doctor will discuss your medical issue and if appropriate your doctor may write a prescription.

NEW! Prescription Advocacy through ImpaxRx

ImpaxRX is a prescription advocacy program that assists employees and their family members with the high cost of certain eligible medications. If the medications you or a family member are taking is identified as eligible under the Medication Under Management (MUM) program, an ImpaxRX advocate will reach out to you to confirm your enrollment in the program and obtain additional information needed to secure the medication at no cost.

Timely participation with ImpaxRX is required to receive these programs for your high-cost medications. Please respond if ImpaxRX contacts you! This will allow them to work on your behalf to obtain you medication at no cost.

There is no cost to you to participate in this program, and there will be no copayments for eligible medications approved by the program. Additionally, by using the program, the cost of these medications is lowered under the Gahanna Health Plan which allows for savings to keep employee premium contributions to a minimum for all.

Dependent Care FSA Flyer

Allows you to pay for eligible dependent care expenses with tax-free dollars so that you and your spouse can work or attend school Full-time.

Funds in a Dependent Care FSA are only available once they have been deposited into your account and you cannot use the funds ahead of time.

  • You may set aside up to $7,500 annually in pre-tax dollars, or $2,500 if you are married and file taxes separately from your spouse.
  • If you participate in a Dependent Care FSA, you cannot apply the same expenses for a dependent care tax credit when you file your income taxes.

Dependent Care FSA funds can be used to pay for things such as:

  • Child care
  • Before-school/after-school care
  • Day camp
  • Dependent care center
  • Nursery school
  • Preschool/Pre-Kindergarten

The City of Gahanna offers 1 medical plan option through UMR.

  1. HDHP Plan: $3,400 Single Deductible / $6,800 Family Deductible

Utilizing In-Network providers will allow for the highest level of coverage. In-Network providers agree to accept UMRs contract rate as the final charge and members are not balance billed. Employees who receive care from an Out of Network provider will be responsible for higher costs and can be balance billed.

*Embedded Deductible: Each person on the plan has their own deductible, however the family has its own total deductible. No single individual on the family plan will need to pay a deductible higher than the individual amount.

Health Savings Account (HSA)

A Health Savings Account (HSA) is a tax-free savings account that is owned by you, is 100% vested from day one, and lets you build up savings for future needs. The funds may be used to pay for qualifying healthcare expenses not covered by insurance or any other plan for yourself, your spouse, or tax dependents. You decide how much you would like to contribute, when and how to spend the money on eligible expenses, and how to invest the balance.

To have an HSA and make contributions to the account you must meet several basic qualifications.

  • You must have coverage under a qualified High Deductible Health Plan (HDHP)
  • Participants cannot be covered by any other health insurance plan (this exclusion does not apply to dental, vision, disability or long-term care coverage)
  • Participants cannot participate in a Healthcare FSA or spouse/domestic partner's Healthcare FSA or Health Reimbursement Account (HRA)
  • Participant cannot be enrolled in Medicare or Medicaid
  • You cannot be eligible to be claimed as a dependent on someone else's tax return
  • You have not received Department of Veterans Affairs Medical benefits in the past 90 days.

An HSA account will need to be established with the banking institution of your choice. The account details will need to be provided to City of Gahanna in order to have the employer contribution deposited into your account.

Maximum contributions are as follows: (new 2026 IRS limits)

  • $4,400 for Employee Only
  • $8,750 for a Two-Person or Family

Please notify HR annually if you'd like your annual service credits to be added to your HSA.

Effective 1/1/2026, RxBenefits has taken on the role of customer support provider for City of Gahanna pharmacy benefits.

Employees and their family members should contact RxBenefits directly for any questions, concerns, appeals, or issue resolution related to their pharmacy benefits.

WHERE CAN I FIND A DRUG LIST?

Typically, a full listing of covered drugs is found on your provider’s website. A drug list, also called a formulary, is a list of generic and brand-name drugs covered by a health plan. Although a drug may be on the drug list, it might not be covered under every plan. Review the plan materials for details on specific benefits.

You can use drug lists to see if a medication is covered by your health insurance plan. You can also find out if the medication is available as a generic, needs prior authorization, has quantity limits and more.

TRADITIONAL DRUGS

TIER 1 (GENERIC) Lowest copay: Most drugs in this category are generic drugs. Members pay the lowest copay for generics, making these drugs the most cost-effective option for treatment.

TIER 2 Higher copay: This category includes preferred, brand name drugs that don't yet have a generic equivalent. These drugs are more expensive than generics, and a higher copay.

TIER 3 Highest copay: In this category are nonpreferred brand name drugs for which there is either a generic alternative or a more cost-effective preferred brand. These drugs have the highest copay.

City of Gahanna dental benefits are offered through MetLife.

Network: PDP Plus (Preferred Dental Program)

Dental benefits through MetLife provides comprehensive coverage to help you and your family maintain good dental health. Your coverage will be greater when you visit a participating In-Network dentist. You will have lower out of pocket costs, no balance billing*, and claims will be submitted by your dentist on your behalf.

Employees who receive care from an Out of Network provider may be responsible for higher costs and may be balance billed.

*Balance Billing: Some Out of Network providers bill a patient the difference between the total cost of the services and the amount insurance paid.

Vision

City of Gahanna vision benefits are offered through MetLife.

Network: VSP Choice

Vision coverage through MetLife helps pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations.

Life Insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. Accidental Death & Dismemberment (AD&D) insurance is equal to your Life benefit in the event of your death being a result of an accident and may also pay benefits for certain injuries sustained.

Voluntary - Employee Paid Benefit through MetLife

Coverage is also available for your spouse and/or child dependents, but only after you've elected coverage for yourself. A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage. Review the attached full benefit summary below for details on reductions, portability and conversion options.

Employee: $10,000 increments up to a maximum of $500,000

Spouse: $5,000 increments up to a maximum of $250,000

Short Term Disability

Short Term Disability (STD) insurance helps you replace a portion of your income during the initial weeks of a disability. STD insurance is now offered to all full-time employees, working at least 20 hours per week.

Critical Illness

Critical Illness Insurance pays money directly to you when you are diagnosed with certain serious illnesses.

This is a voluntary benefit, 100% employee paid, through MetLife. To enroll in Critical Illness coverage please complete the election form below and return to HR.

What's covered?

  • Critical Illnesses such as heart attack, stroke, and end-stage kidney failure
  • Cancer conditions, such as breast cancer and skin cancer
  • Progressive diseases such as Alzheimer's Multiple Sclerosis and Parkinson's disease

Accident Protection

A serious injury can cost you a lot of money – not only in medical bills but also in income from lost work hours. Some injuries are minor, but others are debilitating and require significant medical care. If you get hurt, accident insurance pays you money that you can use to cover personal expenses, bills, and out-of-pocket medical costs. Accident Insurance pays a set benefit amount based on the type of injury you have and the type of treatment you need. It covers accidents that occur on and off the job. And it includes a range of incidents, from common injuries to more serious events.

This is a voluntary benefit, 100% employee paid, through MetLife.

Who Gets Paid?

  • You get paid! When you experience an accident, your health insurance company pays your doctor or hospital, your MetLife accident insurance pays you directly.
  • You can use the money however you want.

Organized Sports Activity Benefit

This coverage includes an Organized Sports Activity Benefit Rider. The rider increases the amount payable for certain benefits by 25% for injuries resulting from an accident that occurred while participating as a player in an organized sports activity. The rider sets forth terms, conditions and limitations, including the covered persons to whom the rider applies
