PPO HSA 3000
In and Out of Network Coverage available
Member is not required to have a designated PCP to receive in-network benefits.
No referral is required to see a specialist.
POS 3000
In and Out of Network Coverage available
Michigan Member is required to have a designated PCP to receive in-network benefits.
No referral is required to see a specialist.
POS HSA 2000
In and Out of Network Coverage available
Michigan Member is required to have a designated PCP to receive in-network benefits.
No referral is required to see a specialist.

Visit BCBSM.com under “Find a Doctor”. Set your location and select “PPO Plans” under plans.

POS - Provider Search

Michigan Member: Visit BCBSM.com under “Find a Doctor”. Set your location and select “Blue Care Network (HMO and POS)” under plans.

Member Outside of Michigan: For BlueCard providers, go to provider.bcbs.com to set your location and select “BlueCard Traditional”
