OVERVIEW

Futuramic remains committed to providing a competitive, cost-effective benefit program. Each year Futuramic takes great care and detail in the benefit programs offered to employees.

Below are highlights of the new changes that will be effective January 1, 2024:


  • Medical/Rx will remain with BCBSM, PPO HSA $3000 moving to PPO HSA $3200 due to Federal Regulations, small increase in contributions
  • HSA employer seed money will only be available to new enrollees
  • Dental will remain with Delta Dental, no changes in contributions
  • Vision will remain with NVA, small decrease in contributions
  • Life, AD&D, and Disability coverage will move to Prudential, no benefit changes

ELIGIBILITY

Helping you and your family reach and maintain good health is very important to Futuramic. Futuramic is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Futuramic. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and child(ren) up to age 26.

ENROLLMENT

ENROLLMENT INSTRUCTIONS:


  1. Go to www.FuturamicConnection.com
  2. Enter your User ID (format is case sensitive) : First Initial Upper Case + Last Name Upper Case first letter only + FTE e.g.John Smith = JSmithFTE
  3. Enter your Password: Last 4 of your SSN
  4. Follow instructions and enroll in your benefits
  5. Make sure to save your elections and print your confirmation statement.


The Benefit choices you make during your your open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called Qualifying Life Event. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

BCBSM PROGRAMS

The following programs are available to all BSBCM members.


Blue 365, is a discounts and rewards program offered to all BCBSM members, see link below to take advantage of virtual fitness classes, healthy food, and much more!

Sempre Rx This discount program focuses on certain drugs treating common chronic conditions where following doctors’ orders can improve your health and prevent serious complications.

You may be contacted by BCBSM via phone or mail to sign up for PillarRx, it's important you don't ignore these calls or letters. The team will only contact you when a drug you're taking is now under a 'co-pay assistance program' called PillarRx. If you don't register for PillarRx, which is free to do, you will be asked to pay a much higher copay than you are used to.

HEALTH SAVINGS ACCOUNT

Health Equity


The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.


To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

  • Covered by a Qualified High Deductible Health Plan
  • You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
  • You cannot be covered by Medicare or Medicaid
  • You cannot be eligible to be claimed as a tax dependent
  • You have not received VA benefits in the past 90 days


2024 HSA IRS Maximums are as follows:

$4,150 for a single contract

$8,300 for a two person or family


$1,000 "Catch Up" contribution for those age 55+


Maximums noted above are offset by any employer provided HSA funding.

Futuramic provides a dental plan with comprehensive coverage to help you and your family maintain good dental health. The same two plan options will continue to be offered.

Eligible employees may pick between the High or Low plans. The High plan will provide richer benefits for basic and major services, as well as orthodontia coverage. In order to get the highest level of coverage, and the lowest out of pocket expenses - be sure to visit an in-network dentist.

VISION

National Vision Administrators (NVA)


Futuramic offers vision coverage through NVA to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Review your coverage details in your Benefit Guide.

FLEXIBLE SPENDING ACCOUNT

The Health Care, Limited Purpose, and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


You may contribute as follows:

Health Care OR Limited Purpose FSA

  • Up to $3,050 annually
  • All benefit eligible can participate
  • If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses


Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns

Futuramic provides you with financial protection options for life, accidental death & dismemberment, short-term disability and long-term disability with Prudential.


Core Life/AD&D - Employer Paid

$25,000 Life and Accidental Death & Dismemberment (AD&D) Insurance


Additional Voluntary Life and AD&D - Employee Paid

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in this coverage for yourself, this will be in addition to your employer provided Basic Life coverage. You can also choose to enroll in coverage for your spouse and dependents.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short-Term Disability - Employee Paid

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.


Long-Term Disability - Employer Paid

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.
