Wiseway is pleased to offer an comprehensive benefits package designed with your health and financial well-being in mind. We review our benefit program each year to ensure our offerings provide the best combination of benefit coverage, network access and affordability.


Eligibility

If you are eligible, you may enroll in benefits the first of the month following 60 days of employment. If you previously waived any benefits, you have the option during Open Enrollment to enroll or make benefit changes and add or drop dependent coverage without having a qualifying event.


You are eligible for the Medical Plan if you work 30 or more hours per week. You are eligible for all other benefits if you work 40 hours per week.

An Overview of 2022 Benefit Changes

Wiseway is pleased to announce that there are no changes to your medical, dental, vision and basic life plans for 2022!


Benefits that are changing include:

  • Your life insurance carrier, Cigna, is now known as New York Life
  • The medical and dependent care flexible spending accounts are being discontinued for 2022. If you are a current participant, you can submit claims for reimbursement within 60 days after the end of the year.
  • Voluntary Life Insurance is new for 2022. You can elect for yourself, spouse and/or child(ren) through New York Life (formerly Cigna)


Your Employee Benefit Guide and this web page provides general information as an overview. If there is any discrepancy between the insurance carrier's certificate of coverage or summary plan description and this web page or Benefit Guide, the insurance plan documents will prevail. ENROLLMENT

How To Enroll In Benefits

Wiseway will conduct a "passive" enrollment this year. This means that your current enrollments will automatically rollover into 2022, with the exception of Flexible Spending Accounts, which will not be offered in 2022.


However, during this open enrollment period, you have the option of making changes to your current elections or enrolling for the first time (if you have met eligibility criteria).

ELECTION CHANGES

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

MEDICAL

Medica

Wiseway offers two medical plan options for employees working 30+ hours per week. All options provide high-quality, affordable medical care, including preventive care, doctor's visits, hospitalization, and emergency care. However, each plan has unique characteristics and advantages. The plans are offered through Medica.


PLAN CHOICES

  • $2,500 Deductible with 80% co-insurance and Rx co-pays
  • $5,250 Deductible with 80% co-insurance (compatible with a Health Savings Account - HSA)


NETWORK

  • Medica Choice Passport with UnitedHealthcare Choice Plus: This network is available for those living inside Medica's service area of Minnesota, North Dakota, South Dakota and western Wisconsin as well as access to providers in a national network through UnitedHealthcare

HSA

Health Savings Account (HSA) Eligibility

Employees who enroll in the $5,250 Plan are eligible to participate in a Health Savings Account (HSA) by establishing an account at a bank of your choice.


Health savings accounts (HSA’s) are a tax-advantaged savings account paired with a high deductible health plan exclusively for the purpose of paying qualified medical, dental and vision expenses for you, your spouse and your dependents.


The IRS limits the amount of contributions allowed each year, as follows:

  • Employee Only: $3,650
  • Employee + 1: $7,300
  • Family: $7,300


If you are between the ages of 55-65, you can also make an additional $1,000 “catch-up” contribution annually.

DENTAL

Delta Dental Plan

Dental Insurance is available to employees working 40 hours per week.


Delta Dental has the largest dental provider network in Minnesota and the United States. The dental plan utilizes two Delta Dental network options and provides you with the freedom to see any dentist you choose.

VISION

EyeMed Vision Plan

The Voluntary Vision plan is available to employees working 40 hours per week.


Vision coverage through EyeMed provides benefits for eye exams, prescription glasses, contact lenses, as well as other vision-related items.


Eyemed's Insight network offers access to a large variety of providers; using an in-network provider increases your discounts for services.

BASIC LIFE/AD&D

Wiseway provides all employees with basic term life and accidental death and dismemberment (AD&D) coverage at no cost to you.


AD&D benefits are payable to you in the event of a dismemberment or an additional benefit is payable to your beneficiary if your death is the result of an accident.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

Life Assistance Program (LAP)

Life. Just when you think you’ve got if figured out, along comes a challenge. Whether your needs are big or small, your Life Assistance & Work/Life Support program is there for you. It can help you and your family find solutions and restore your peace of mind.

 

Your program includes up to three face-to-face assessment and counseling sessions per issue. EAP Services can assist with:

  • Depression, grief, loss and emotional well-being
  • Family, marital and other relationship issues
  • Life improvement and goal-setting
  • Additions such as alcohol and drug abuse

VOLUNTARY LIFE

New for 2022!


Wiseway is pleased to offer the ability for you to elect voluntary life insurance for yourself, your spouse and/or your child(ren).


All active, full-time employees working a minimum of 40 hours per week are eligible. Coverage is effective for new hires the first of the month following 60 days. Spouses are eligible up to age 70, as long as you apply for and are approved for coverage on yourself. Your child(ren are covered from birth to age 26, as long as you apply for and are approved for coverage yourself.


  • Employees may elect a benefit amount in increments of $10,000 up to 5x salary or $500,000. Guaranteed Issue is $100,000*
  • Spouses may elect a benefit amount in increments of $5,000 up to $250,000 not to exceed 50% of the employee's benefit. Guaranteed Issue is $25,000*
  • Children may be covered in increments of $1,000 up to a maximum of $10,000. No statement of health is required. Children under 14 days to 6 months old are eligible for a benefit of $500.


A matching amount of Accidental Death and Dismemberment is included.


*Guaranteed Issue means that you may be able to purchase coverage up to this limit without medical exams or health questions. If you are a new hire and you apply within 31 days after you are eligible to elect coverage for yourself, you are entitled to choose any coverage offered up to the Guaranteed Issue Amount, without providing proof of good health. If you apply for an amount of coverage greater than the Guaranteed Issue Amount, coverage in excess of the Guaranteed Issue Amount will not be issued until the insurance company approves acceptable proof of good health.
