Dental Benefit Overview
You have the option to elect into dental plan administered by Guardian
Single Deductible $0
Family Deductible $0
Preventative Care Plan Pays 100%
Basic Care Plan Pays 80%
Major Care Plan Pays 50%
Annual Plan Maximum $1,000 per person covered under plan
There is no orthodontia coverage

Vision Benefit Overview
You can enroll in the vision plan offered through Guardian using the VSP network.
Your vision benefit through Guardian is described below. Please refer to www.guardiananytime.com for a list of vision providers. Guardian utilizes the VSP network which includes retail providers such as LensCrafters, Pearle Vision, Sears, Target and JCPenney in addition to many independent optometrists and vision providers.
Eye Exam - $10 Copay
Standard Glass Lenses - $$25 Copay
Frames - $25 Copay up to $120 frame, then 20% of remaining frame purchase

Group Basic Life & AD&D
Medical Service Company knows your family depends on you in many ways, including financial security. As an employee of MSC, you are automatically enrolled in a Basic Life Insurance plan. This benefits will pay up to 100% of your annual salary to a maximum of $50,000 should an unforeseen occurs.
Should you and your loved ones feel additional insurance will benefit you, you will have the option to enroll into voluntary life for you, your spouse and or dependent child up to age 23.

Voluntary Short & Long Term Disability Benefit Overview
You have insurance for your car and home, but what about income that pays for it? You rely on your paycheck for many things, but what happens if you are suddenly unable to work due to an accident or illness? Disability insurance can help replace lost income!
Short Term Disability - will provided 60% of your salary to a maximum of $1,500 up to 13 weeks
Long term Disability - will provided 60% of your monthly salary up to $6,000 a month beginning on day 91 from your accident or illness

Voluntary Accident & Critical Illness Benefit Overview
Voluntary Accident with Guardian gives you and your family the peace of mind knowing your are protected.

When you or a family member suffer from a serious illness such as stroke or heart attack, your out of pocket costs add up. Critical Illness Insurance will help with expenses that your medical coverage doesn't cover, such as deductible, out of pockets or experimental treatment.

Your four legged, furry, slithery or scaly family member needs care too! Nationwide can help you ensure they are covered!
