Basic Life/AD&D : Provided through Guardian with no plan changes at no cost to the employee. 100% Employer paid benefit. 1x Salary to $50,000

Voluntary Life/AD&DProvided through Guardian with no plan changes. Employees, spouse & dependent children may elect additional coverage through payroll deductions.

Voluntary Short-Term Disability and Long Term Disability: Provided through Guardian with no plan changes. Provides income replacement if unable to work due to a disability
