The Franklin Building Supply Employee Benefits program offers you core benefits, such as Medical and Dental as well as voluntary Vision, Life, and Disability benefits that help maximize your coverage options. Please note: It is important that you enroll in a timely manner. If you do not enroll within your first 30 days of employment, you will not be eligible to enroll without a qualifying life event until the next open enrollment period.
If you leave Franklin Building Supply, your Medical, Dental, and Vision coverages will terminate on the last day of the month in which you leave your job. For example, if you leave your job on April 16th, your coverage will end on April 30th. These coverages can be continued under COBRA for a period of time at your own expense. You will receive COBRA election paperwork and instructions directly from Paylocity.
Your Voluntary Short-Term Disability, Voluntary Long-Term Disability, and Voluntary Life and AD&D coverages will terminate on your last day of employment. The Voluntary Life and AD&D plan does include portability and conversion rights that allow you to maintain your coverage. Reach out to United Heritage within 60 days of your termination to inquire about your options.
Under certain circumstances, employees may be allowed to make changes to benefit elections during the plan year, if the event affects the employee, spouse, or dependent’s coverage eligibility. Any requested changes must be consistent with and on account of the qualifying event.

DEPENDENT CARE FSA (DCA)

A Dependent Care FSA (DCA) is a reimbursement program that allows employees (participants) to set aside pre-tax funds to help pay for qualified dependent care expenses. Most participants use this program to pay for child daycare and after-school care expenses; however, it can be used to pay for adult daycare expenses as well. It serves as an alternative to using the Dependent Care Tax Credit. Funds can only be used on a dependent child under the age of 13 or dependents who are unable to care for themselves. Unlike a Flexible Spending Account, DCA funds can only be used as they are deposited into your account.

Franklin Building Supply offers you two dental programs: Delta Dental and Willamette Dental. The two plans operate very differently. For example, the Delta Dental plan pays a percentage of covered services up to an annual maximum whereas the Willamette plan has no annual maximum and you pay a set fee for covered services.
