Benefits Overview
United Healthcare (UHC) is the Vendor for the Medical Program
UHC Medical
- Choice / Choice Plus Network
- If you are enrolled in the in-network only EPO $0 Deductible Plan, you will use the Choice Network. All other plan options will use the Choice Plus Network.
- Five plans to choose from
- Three Tier Prescription drug coverage
- Prescription Drug Coverage and Preferred Drug List – with a large national network of chains, independent retail pharmacies and home delivery, and online tools to help you compare and save
- Health and Wellness Resources – online and telephonic resources to help you get, and stay healthy
- Customer Service – you can contact UHC by phone (1.866.414.1959), online at www.myuhc.com, or via Facebook https://www.facebook.com/UnitedHealthcare and Twitter https://twitter.com/uhc whenever you need assistance
How to Search for an In-Network Provider
1. Go to www.myuhc.com
2. Click on “Find a Provider”
3. Select the type of provider you are looking for
4. Click on "Employer and Individual Plans"
5. Click on the Choice / Choice Plus Network
6. You can then either enter your provider name and search, or click on the provider type by the categories listed
United Healthcare (UHC) is the Vendor for the Dental Plans
Dental DPPO and DHMO
- DPPO: two plan options with in and out-of-network coverage.
- DHMO: one plan with in-network only coverage.
United Healthcare (UHC) is the Vendor for the Vision Plans
- High and Low Plan Options with in-network coverage and out-of-network reimbursements are available
Guardian is the Vendor for the Group Life/ AD&D and Voluntary benefits
- Basic Life / AD&D
- We will be offering all eligible employees a $10,000 Life and AD&D benefit at no cost to you. You will have the option to enroll in Supplemental Life / AD&D coverage for yourself and your family; you will be responsible for the cost of this coverage.
- Voluntary Short-Term Disability (STD)\
- STD will be offered on a voluntary basis, and employees who elect will be eligible for the following benefits:
- After being continuously disabled for 8 days for a sickness or illness, the plan will pay 60% of your weekly earnings up to a maximum benefit of $1,000 for 26 weeks.
- Voluntary Long-Term Disability (LTD)
- LTD will be offered on a voluntary basis, and employees who elect will be eligible for the following benefits:
- After being continuously disabled for 180 days, the plan will pay 60% of your monthly earnings up to a maximum benefit of $5,000. The benefit will continue to be paid until you are no longer disabled or reach normal social security normal retirement age.
- Voluntary Worksite Benefits
- Details regarding the Voluntary Accident, Critical Illness, Hospital Indemnity, and Pet Insurance plans are available in the Enrollment Guide or in the sections below.
How To Enroll
If you are a new hire or experience a life change event, enrollment will be completed through the Paylocity portal.
- New Hires:
- You will enroll through the Paylocity portal, new hires will receive an email prompting a first-time login. Follow the link provided in your email to access the Paylocity enrollment site. Contact your human resources department directly if you have questions about how to access your account.
- Once logged in, you may start the process of electing benefits.
- Qualifying Life Events (please refer to the Benefits Guide in the Benefits Overview section to find out more about Life Events).
- If you experience a life change event and need to update your benefits elections, you will do so by using your current log in credentials through the Paylocity portal.
Medical Insurance
This section provides information on The Company's medical plans and resources through UHC available to employees. The Company offers five different medical plans.
Dental Insurance
This section provides information on The Company's dental plans and resources through UHC available to employees. The Company offers three different dental plans.
- Note the DMO plan is not available in MA, ME, MN, NC, NM
Vision Insurance
This section provides information on The Company's vision plans and resources through UHC available to employees. The Company offers two different vision plans.
Health Savings Account (HSA)
If you enroll in the HDHP (HSA), a health savings account with Optum bank will automatically be opened for you and you will receive a Welcome Packet and debit card in the mail to use on eligible purchases. For 2023, the maximum tax-deductible contribution is $3,850 for an individual and $7,750 for all other coverage levels.
Flexible Spending Accounts & Commuter Benefit Plans
This section provides information on The Company's flexible spending account(s) and resources through Paylocity available to employees.
Voluntary Short-Term Disability and Long-Term Disability
This section provides information on The Company's Voluntary Short-Term Disability and Voluntary Long-Term Disability through Guardian available to employees. If you choose to enroll in either or both coverages, you will be responsible for 100% of the cost.
Basic and Supplemental Life / AD&D
In addition to the Basic Life / AD&D Insurance that The Company provides to employees at no cost, there is an option to supplement this coverage by enrolling in the Supplemental Life / AD&D Insurance plans. There are Supplemental Life / AD&D Insurance plans available for your spouse and child, as well.
- Please be sure your Life/ AD&D and Supplemental Life/ AD&D lists the correct beneficiary to whom your benefits should be paid to if utilized.
Employee Assistance Program (EAP)
This section provides information on The Company's Employee Assistance Program through Guardian available to all employees. This comprehensive program provides support for:

401k
The Company offers employees the opportunity to save for their retirement. Information in this section provides details on the Companies John Hancock 401k plan and how to enroll. Accident, Critical Illness, & Hospital Indemnity
The Company offers additional coverage through Voluntary Benefit Plans where you are responsible for 100% of the cost. These plans include Guardian Accident, Critical Illness and Hospital Indemnity for you and your family, which you can enroll in through the Paylocity portal.
Pet Insurance
This section provides information on The Company's pet insurance plans through Pets Best.

Note: Pets Best is currently unavailable for CA residents. If you reside in CA and would like to purchase pet insurance, please enroll in an ASPCA pet plan through the InsurChoice portal in the section below.
