2022 Open Enrollment
Floorplan Xpress is pleased to announce the 2022 Annual Open Enrollment period. This is the time each year when employees are encouraged to take the opportunity to re-evaluate current and future benefit needs. Understanding your 2022 benefit plan options is important, so you are encouraged to review all information carefully. You may also want to visit the carrier websites to take advantage of the many tools and resources available to assist you in making a well informed decision.

This is your time to select your benefit choices. Please review the following information:

How To Enroll
To enroll in the 2022 Benefits, please complete the following:
- Download the 2022 Floorplan Xpress Medical, Dental & Vision Open Enrollment Form.
- Download the 2022 Mutual of Omaha Enrollment Form.
- Download the 2022 Flexible Spending Account Enrollment Form
- If you elected the BCBS HDHP Medical Plan and wish to open a Health Savings Account, please download the 2022 HSA Enrollment Form.
- Complete all forms, sign, date and return to Dusty Webb, Director of Human Resources via Teams.
- Everyone must complete and submit benefit elections to Human Resources by December 13, 2021.
- To participate in a Health Savings Account or Flexible Spending Account in 2022, you must make an active election by December 13, 2021.

While we are interested and concerned about the health of our clients and their dependents, we have HIPAA Privacy Compliance obligations which limit the information carriers can share with us. NFP is available to assist you with claims issues or any other issues you have concerning our benefits. If you are experiencing a claim issue, please contact Customer Service with the appropriate insurance company first to see if you can resolve it on your own.
