Please follow the steps below to login to ADP to review your current elections and make any changes if you experience a Qualifying Life Event. If you need assistance with logging in, please reach out to HR.
Benefit changes outside of Open Enrollment require a Qualifying Life Event and/or Loss of Eligibility.
You may not change your benefits throughout the year unless you experience a Qualifying Life Event. Qualifying Events may include marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you must notify HR within 30 days of the event; you will then be able to log in to ADP and make changes to your benefits. Please remember you only have 30 days from the event to make changes to your benefit elections.
Blue Cross and Blue Shield of Oklahoma will continue to administer our medical and prescription drug benefits.

Pharmacy benefits are through your BCBS medical plan.

Keystone administers our Flexible Spending Account benefit.
What is a Medical FSA?
A Medical FSA allows you to choose how much of your paycheck you'd like to set aside for Medical, Dental and/or Vision expenses. FSA dollars can be used throughout the year to pay for qualified expenses and services.
What is a Limited Purpose FSA?
A Limited Purpose FSA is a benefit that allows you to choose how much of your paycheck you'd like to set aside for qualified dental, vision and preventative care expenses only. A Limited Purpose FSA works with a high-deductible health plan and Health Savings Account. If you choose to participate in the Health Savings Account, you are only allowed to participate in the Limited Purpose FSA, not the Medical FSA.
Lincoln Financial Group will continue to administer our Health Savings Account.
Below are the 2026 HSA contribution limits as well as the amounts FSB will contribute to your Health Savings Account per pay period.
You will use your retirement plan account username and password to login to the Lincoln Financial employee portal. If you do not have a login, register by clicking on "register now".
Your dental coverage is offered through SunLife.

Your vision coverage is through Ameritas.
Ameritas utilizes the VSP network. To find an eye doctor, please visit the "VSP Find an Eye Doctor" link below.
FSB provides all full-time employees with a minimum of $40,000 of Basic Life/AD&D coverage. This will be provided through Prudential.
Who's Your Beneficiary? Everyone needs to make sure your beneficiary is up to date.
Don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child. If you are needing to update your beneficiary mid-year, please notify HR and login to your ADP profile to update.
You have the opportunity to enroll in Optional Life and AD&D in addition to your Basic Life coverage provided by FSB. Coverage is also available for your spouse and/or child dependents. You are required to elect coverage for yourself in order to be eligible for coverage on your dependents.
Short-Term and Long-Term Disability will remain no cost to you.
Accident insurance provides you with the extra money you need to help cover the incurred expenses, medical or otherwise, you face when you suffer an injury due to an accident.

If you, or a covered depended, are diagnosed with a covered critical illness this policy will pay you a lump sum for you to use as needed. Premium is calculated on age and benefit amount elected.

Hospital Indemnity insurance pays you fixed dollar amounts regardless of the amount that the provider charges. These benefits are paid in addition to any other health coverage you may have.

Your Identity Theft Protection coverage is administered by Norton LifeLock.
Your 401(k) plan is administered by Lincoln Financial Group.
You will automatically be enrolled in the 401(k) plan on your hire date for 4% of your salary. FSB will match up to 50% of your employee contribution and will be capped at 5% of your monthly salary.
When will my 401(k) changes take effect?
If you make changes to your 401(k) contribution through Lincoln, be sure to submit them at least one week before your next payday (15th or the last day of the month) to ensure they show up on that payroll. Changes submitted after may not be captured in time and will instead take effect on the following pay period.

Employee Assistance Program - The GuidanceResources Program offered through Prudential is available to you and your dependents, at no cost, to provide confidential support, resources and information to get through life's challenges. You can call anytime with personal concerns such as grief and loss, depression or stress and anxiety.
Travel Assist Program - In a medical emergency, this benefit will coordinate and provide Emergency Medical Transport services to help you get the care you need. These services can include things such as an emergency medical evacuation, visit of a family member or friend, emergency hospitalization and much more.
