Welcome
Helping you and your family reach and maintain good health is very important to FEV North America, Inc. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Wellness
Enrollment
All regular employees working at least 30 hours per week are eligible to participate in the benefit plans offered by FEV. Your coverage becomes effective the first of the month following your date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Children eligibility is as follows -
- Medical, Dental & Vision: End of the year the child turns 26
- Voluntary Life, Accident, Critical Illness: Age 19 or 26 if child(ren) is a full-time student
ENROLLMENT INSTRUCTIONS:
- Go to www.http://fevna.bswift.com.com (accessible via "Enroll Here!" button below)
- Follow Instructions and enroll in your benefits
- Make sure to save your elections and print your confirmation statement
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

The HMO plan is for Michigan resident employees only. Out of State employee will need to choose one of the two PPO plans.
**If you enroll in the HMO you must complete a health assessment and qualification form within first 90 days of your plan start date (12/1/2022). The steps to receive the “Enhanced” plan design benefits are included in the benefit guide. If you do not complete these, you will receive the “Standard” plan design benefits.
Medical Plan Options Considerations
MEDICAL OPT-OUT PAYMENT
A $1,200 opt-out payment (divided between pay periods annually) is available if you provide proof of other coverage
WORKING SPOUSE ACCESS FEE
A $1,200 (pre-tax, divided between pay periods) will be accessed if your working spouse:
- Is eligible for group medical coverage through his/her employer; and
- Is covered as your dependent on the medical plan provided by FEV
EMPLOYER HSA CONTRIBUTION
A $250 (Single) or $500 (Two Person or Family) employer contribution will be made to your Health Savings Account if you elect the PPO HSA. This is funded quarterly and will be prorated depending upon your benefit start date.

Health Savings Account

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
- Covered by a Qualified High Deductible Health Plan
- You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
- You cannot be covered by Medicare or Medicaid
- You cannot be eligible to be claimed as a tax dependent
- You have not received VA benefits in the past 90 day

Flexible Spending Accounts

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.

Health Care FSA
$2,850 Max. Contribution
- Use to cover your out-of-pocket costs for qualified medical, dental and vision expenses for yourself, spouse and dependent child(ren)
- Full amount elected is available Day 1
- At the end of each plan year, you may use these funds left over from the current year FSA balance to pay for eligible expenses incurred in the first 2 ½ months of the following plan year.
- Any Unused funds beyond the 2 ½ month grace period will be forfeited.
Limited Purpose FSA
$2,850 Max. Contribution
- If you are enrolled in the HDHP and set up an HSA, you can ONLY participate in a Limited-Purpose FSA
- Use to cover your out-of-pocket costs for qualified dental and vision expenses for yourself, spouse and dependent child(ren)
- Full amount elected is available Day 1
- At the end of each plan year, you may use these funds left over from the current year FSA balance to pay for eligible expenses incurred in the first 2 ½ months of the following plan year.
- Any Unused funds beyond the 2 ½ month grace period will be forfeited.
Dependent Care FSA
$5,000 Max. Contribution ($2,500 if married, filing separately)
- Use to cover day-care expenses for qualified dependents such as pre-school programs, Latch Key programs through age 13, Daycare Centers for Adults
- Only have access to the account balance
- At the end of each plan year, you may use these funds left over from the current year FSA balance to pay for eligible expenses incurred in the first 2 ½ months of the following plan year.
- Any Unused funds beyond the 2 ½ month grace period will be forfeited.

We are proud to provide all team members with Basic & Voluntary Life/AD&D coverage as well as Short- and Long-Term Disability through UNUM.
Basic Life/AD&D
Employees working 30 or more hours per week are eligible for twice your salary, to a maximum of $200,000, in coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.
Voluntary Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Short-Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short-Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage through UNUM pays 60% of your income to a maximum of $1,846 per week, for up to 26 weeks at no cost to you!
Long-Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long-Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a 180-day elimination period are equal to 60% of covered monthly earnings up to $8,000 for as long as you are disabled or until you reach Retirement Age whichever is sooner.

401k
The FEV North America, Inc. 401K Plan is available to all employees who meet the requirements of the plan. It makes saving for retirement easy and convenient, and helps you secure your financial future. Electing to contribute money into the plan also lowers your taxable income so you pay less in taxes now and take more income home. You do not pay taxes on your savings while your contributions remain in your account. FEV matches your contribution dollar for dollar up to a limit of 5%.
FEV’s 401(k) plan is administered by Principal. Use link below to log into Principal to access your account, update your contributions and investment options, obtain information about loans, withdrawals, roll overs and other planning resources. You may also call Principal at 800-547-7754.
Eligible employees are auto-enrolled in the plan at a predetermined minimum contribution percentage unless they go to the Principal website and select another percentage and/or opt-out entirely. At the date of hire, the predetermined minimum contribution percentage is 6%. At the beginning of each calendar year, an additional 2% is added to each employee’s contribution percentage unless the employee has opted-out of the auto escalation. The auto escalation will max out at 12%.
