Introduction


 At FBLA, we care about the welfare of our employees and want to ensure that you have comprehensive insurance coverage to carry you and your family through life’s expected and unexpected events. You should carefully consider your enrollment decisions because your election remains in effect for your entire plan year (September 1 through August 31). After you make your elections, the only time you can make changes to your benefits programs during the plan year is if you experience a special enrollment situation or a change in family status. Otherwise, no changes can be made.

ACTION REQUIRED

Complete the Employee Enrollment Change form if you need to:

  • Enroll in new benefits
  • Make changes to your current benefits elections
  • Qualifying Life Changes:
  • Documentation of QLE is required
  • Must notify your HR Dept within 30 days of the event
  • For a list of Life Events click Here
  • Update your beneficiary information
  • Completed form must be submitted to Robert Holst
