Helping you and your family reach and maintain good health is very important to Alline Salon Group. Alline Salon Group is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Alline Salon Group. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse, domestic partner, and child(ren) up to age 26.


In order to be eligible to cover a same or opposite domestic partner, you must have cohabitated for a minimum of 6 months; proof of this may be requested.



OVERVIEW

Alline Salon Group remains committed to providing a competitive, cost-effective benefit program. Each year Alline Salon Group takes great care and detail in the benefit programs offered to employees. Below are highlights of the new changes that will be effective January 1, 2022:


  • Medical plans will now include new Point of Service options
  • Dental insurance will now be provided with Delta Dental

ENROLLMENT

How To Enroll In Benefits

Action Required:


All elections must be submitted by November 30, 2021.


ENROLLMENT INSTRUCTIONS:

  1. Sign in through www.paycor.com, then hover over ME and select, “BENEFITS”.
  2. This will take you directly to the Benefits Advisor site. You can review the details of each plan by visiting the ‘LIBRARY’ tab at the top of each page and select ‘CONTENT’
  3. After you've reviewed the offered plans and made your decisions, click on “START ENROLLMENT”
  4. Your current enrolled, waived, and past benefit elections will be displayed here.
  5. You will need to either ENROLL or WAIVE each benefit selection.


The Benefit choices you make during your your open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Alline Salon Group provides those employees who are covered by one of the BCBSM/BCN medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

DENTAL

Delta Dental Plan

Alline Salon Group's dental plans provide comprehensive coverage to help you and your family maintain good dental health. For 2022, the new dental carrier will be Delta Dental. The same two plan options will continue to be offerred.

Eligible employees may pick between the Basic or Enhanced plans. The Enhanced plan will provide richer benefits, orthodontia coverage, as well as both Delta Dental PPO and Premier dentists being considered as in-network dentists. The Base plan is limited to include Delta Dental PPO dentists as in-network.

  • Babies should receive their first professional eye exam at 6 months

Alline Salon Group offers vision coverage through National Vision Administrators (NVA) to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Alline Salon Group provides you with financial protection options for life, accidental death & dismemberment, short term disability and long therm disability with Unum.


Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.

Please note - if you (or your spouse) did not enroll in coverage when you first eligible, you will be subject to Evidence of Insurability where Unum will review your medical status.

FSA

Flexible Spending Accounts

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


New for 2022 - those who elect the Dependent Care FSA will now receive a debit card.


Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


You may contribute as follows:

Health Care FSA

  • Up to $2,750 annually
  • All benefit eligible can participate
  • If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses


Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.


To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

  • Covered by a Qualified High Deductible Health Plan
  • You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
  • You cannot be covered by Medicare or Medicaid
  • You cannot be eligible to be claimed as a tax dependent
  • You have not received VA benefits in the past 90 days


HSA IRS Maximums are as follows:

$3,650 for a single contract

$7,300 for a two person or family


$1,000 "Catch Up" contribution for those age 55+
