  • If you'd like to change your existing coverage or make new elections, you must do so by January 14.
  • Your current medical election(s) will carry forward to 2022.
  • **New for 2022 - If you wish to enroll in dental and/or vision please complete the UPMC Enrollment Change Form attached.

NEW HIRE ENROLLMENT INFORMATION

If you are a NEW HIRE and need to enroll for the FIRST time please see above section How to Enroll.

All enrollment forms are attached for download.

If you enroll in the UPMC medical you will automatically be enrolled in the supplemental medical for the Gilsbar Gap Plan.
