The Excel Health plans are fully insured medical plans administered by Assured Benefits Administrators and have a Vanishing Deductible!
NOTE: The IRS limits the disappearing deductible on the High Deductible Plan. The disappearing deductible program can only be waived down to the IRS allowable minimum of:
Individual $1,500
Family $3,000
Dental Select's dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Dental Select Platinum Co-Insurance network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher. To view your paystubs or W2 you need to sign into your Paylocity account. Upon hire, you should receive a welcome email sent to the email provided on the employee intake form. If you did not receive an email, please contact your manager and they will have one resent.
