To enroll in or make changes to benefits with United Healthcare, please complete the enrollment/change form.


NOTE:

  • As a new hire, coverage will be effective on the first day of the month following 60 days of employment
  • Outside of the yearly open enrollment, changes to coverage can only be made when experiencing a qualifying event (marriage, divorce, birth of a child, etc.)
