Introduction
Open Enrollment is your annual opportunity to review your benefit elections and make any needed changes for the upcoming year. We are pleased to announce that we are renewing medical coverage through Blue Cross Blue Shield of IL as well as adding dental and vision coverage through Blue Cross Blue Shield of IL. This will take place of your previous dental and vision coverage offered through Unum. Our benefit period is March 1st, 2026 - February 28th, 2027.
Important Things to Know:
- If you are a full-time eligible employee and wish to enroll in coverage, drop coverage, change plans, or add/remove a dependent, please complete the applicable enrollment form.
- If you were previously enrolled in dental and vision coverage, please complete the applicable enrollment form for new plan elections.
- You will not be able to make changes outside of open enrollment unless you have a qualifying event (marriage, birth, divorce, etc).
- All changes will be effective March 1st, 2026.

Upon enrollment in benefits with Eden Supportive Living, you will have access to a variety of employee resources & value adds.
Members enrolled in the HMO plan are eligible for Wondr Health and the Employee Assistance Program (EAP) through ComPsych only.
