Open Enrollment is your annual opportunity to review your benefit elections and make any needed changes for the upcoming year. We are pleased to announce that we are renewing medical coverage through Blue Cross Blue Shield IL as well as dental and vision coverage though Unum. Our benefit period is March 1st, 2024 - February 28th, 2025. Open enrollment will be held from February 5th through February 16th.

If you are a full-time eligible employee and wish to enroll in coverage, drop coverage, change plans, or add/remove a dependent, please complete the applicable enrollment form.
If you are not making any changes to your current benefit elections, you do not need to complete any forms. Your current coverage will be renewed.
Upon enrollment in medical benefits with Eden Supportive Living, you will have access to a variety of employee resources & value adds.
Members enrolled in the HMO plan are eligible for Wondr Health and the Employee Assistance Program (EAP) through ComPsych only.
