Eligibility
Employees working at least 30 hours per week are eligible for benefits. During enrollment, you may also cover your eligible dependents, such as legal spouse and/or child(ren) up to age 26.
Effective Date
Benefits start on the first of the month after your hire date. If you are hired on the first of the month, your benefits will begin that same day.

The benefit choices you make during your initial enrollment or annual open enrollment will remain in effect for the entire year.
You can, however, modify your elections under certain circumstances, called "Qualifying Events". Qualifying events include:

BCBSND will manage your medical and prescription drug benefits. We offer one medical plan option:
UNUM's dental plan provides comprehensive coverage to help you and your family. You can use network dentists or go out-of-network, where you will be reimbursed at the 90th percentile of usual and customary fees.
Avesis coverage helps you pay for eye exams, prescription glasses, sunglasses and contact lenses. You receive a higher level of benefits when you see a provider in the Avesis network.
Direct Auto offers company-paid Basic Life, Basic Accidental Death & Dismemberment (AD&D), Short-Term Disability, and Long-Term Disability coverage through UNUM.
Basic Life and AD&D
Employees working 30+ hours per week are eligible for life insurance coverage of 2.5 times their annual earnings (up to $500,000) payable to a designated beneficiary upon death. The AD&D benefit is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.
Short-Term Disability
Everyday illnesses or injuries can disrupt your work and complicate household expenses. Short-Term Disability insurance helps by providing financial support, allowing you to focus on recovery. If you're eligible and approved, coverage starts after a 7-day absence, offering 60% of your income, up to $2,000 per week, for a maximum of 12 weeks.
Long-Term Disability
Serious illnesses or accidents can disrupt your life and prevent you from working for months or even years. Long-Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. If you qualify, benefits start after a 90-day waiting period and cover 60% of your earnings, up to a maximum of $10,000 per month.
The Dependent Care Flexible Spending Account (FSA) lets you set aside pre-tax money for eligible dependent expenses, helping you save on taxes.

The amount you choose during enrollment is your contribution for the whole year. You can only change it if you have a qualifying life event.

How the Plan Works:
Eligible expenses must occur between January 1 and December 31. An expense is considered incurred when the service is performed, not when you pay for it. You have until March 31 to file your claims. Any unclaimed funds after that date will be forfeited.
The Health Savings Account (HSA) lets you set aside pre-tax money for eligible expenses for yourself and your dependents, reducing your taxable income and saving you money on taxes.

Voluntary Life and AD&D Coverage:
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and AD&D insurance for yourself, your spouse, and your children, supplementing your employer-paid Basic Life coverage. An Evidence of Insurability form may be required for enrollment.
This employer-sponsored retirement savings plan works by taking a percentage of employees' salaries and placing it in investments.
Employee Contributions
Effective 1/1/2025, employees are automatically enrolled in the 401(k) plan at 6% of their pay, unless you choose a different amount or decide not to defer. You can select either Pre-Tax or Roth deferrals. You may defer up to 80% of your pay or $23,500 in 2025. If you are age 50 or older by year-end, you can contribute up to $31,000. Contributions can be changed anytime online with the Principal.
Employer Match
To be eligible for the company match, you must be employed for one year, be an active employee on the last day of the plan year, worked at least 1,000 hours during that year, and be 21 years of age or older. Direct Auto will match 50 cents of every dollar up to 6% of compensation.
UNUM's Employee Assistance Program (EAP) is a free, confidential service available to help employees and their families with personal or work-related issues affecting job performance and well-being.
