Helping you and your family reach and maintain good health is very important to Dahl Trucking LLC. Dahl Trucking is pleased to offer a selection of benefits designed with your health and wellbeing in mind. We review our benefit package each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

Eligibility

Employees working at least 30 hours per week are eligible to participate in benefits starting on the first of the month following 60 days of employment. You must enroll within 30 days of your eligibility date. When you enroll in the benefits package, you may also cover your eligible dependents. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

How To Enroll In Benefits

The benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.

You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

MEDICAL/RX

Medica

Medical and prescription drug coverage is provided through Medica. We offer:

  • $3300 - 0% HSA
  • $6350 -0% HSA
  • $7050 - 0% HSA

Once you select the deductible option you prefer, you have the option of selecting from the below networks:

  •  Medica Passport (open access network) 
  • Medica Altru (Altru Only Network) 

Dahl will contribute towards employee medical premiums as follows:

  • Year 1 – Dahl will contribute 400.00
  • Year 2 – Dahl will contribute 500.00
  • Year 3 – Dahl will cover 100%

DENTAL

Dental coverage is offered through Principal.

Dahl Trucking's dental plan provides comprehensive coverage to help you and your family maintain good dental health. 

  •  $50 single/$150 family deductible, 4 cleanings, $2,000 annual max, adult/child orthodontic coverage with $1,500 lifetime ortho max

VISION

Principal

Dahl Trucking offers vision coverage through Principal to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the Principal VSP Vision Network.

  • Exams Every 12 months, Contacts Every 12 Months, Frames Every 24 Months 
  • $150 Frame and Contact Allowance 
  • $60 Copay - contact fitting and evaluation
  • $25 Copay - prescription glasses

HSA

Lively

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.

To be eligible for an HSA, you must be enrolled in a High Deductible Health Plan (HDHP). You're not eligible if:

  • You are claimed as a dependent on someone else's tax return
  • Covered by another plan that conflicts with the HDHP (Medicare, Veteran's or Tribal benefits, medical flexible spending account, or select Health Reimbursement Arrangements)

2025 IRS Maximums (employee + employer contributions):

  • $4,300 for single coverage
  • $8,550 for family coverage
  • $1,000 catch-up contribution for those age 55+

Please note that Dahl Trucking does not currently provide employer contributions to the Health Savings Account.
