BCBSIL ENROLLMENT INSTRUCTIONS:
If you wish to make changes to your current BCBSIL benefits, or would like to make a new election, please complete the BCBSIL employee enrollment & change form.
FLEXIBLE BENEFITS HRA ENROLLMENT INSTRUCTIONS:
To take advantage of the Digi Trax HRA, please complete the Flexible Benefits employee enrollment form when you enroll in medical coverage with Blue Cross Blue Shield of Illinois. This only applies to members electing the S534PPO plan.
GUARDIAN ENROLLMENT INSTRUCTIONS:
If you wish to make changes to your current Guardian benefits, or would like to make a new election, please complete the Guardian employee enrollment & change form.
UNUM ENROLLMENT INSTRUCTIONS:
If you are a new hire, you will need to complete the UNUM enrollment form. If you are currently enrolled in UNUM LTD coverage and wish to update beneficiary information, please complete the enrollment form.
SONTIQ IDENTITY THEFT ENROLLMENT INSTRUCTIONS:
Digi Trax is offering identity theft protection benefits to all full-time employees at no cost. Please complete the Sontiq enrollment form.
