Helpful Tips

Helpful Tips To Consider Before You Enroll

Do you plan to enroll an eligible dependent(s)?

If so, make sure to have their social security numbers and birthdates available. You cannot enroll your dependent(s) without this information.

Have you recently been married/divorced or had a baby

If so, remember to add or remove any dependent(s) and/or update your beneficiary designation.

Did any of your covered children reach their 26th birthday this year?

If so, they may no longer be eligible for benefits, unless they meet specific criteria.

How To Enroll In Benefits

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.

You can, however, modify your elections under certain circumstances, called "Qualifying Events". These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

You are eligible to participate if you are full-time and work a minimum of 30 hours per week. Your coverage will be effective 1st of the month following date of hire.

All employees need to complete their elections through Proliant by 02-23-2022.

Employer Paid and Voluntary Life offered through Principal

Life insurance can provide your dependents with financial security, and upon your death, can be used for any expenses that could burden your family.

Employer Paid:

Driftwood provides Basic Life and Accidental Death and Dismemberment (AD&D) insurance in the amount of $50,000 to full-time employees at no cost to you.

Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.

At Open Enrollment, all employees have the option to increase their voluntary life coverage up to two increments even if you have previously waived coverage.

Disability offered through Principal
