If you have gone through open enrollment before, you will see that there were not any changes to your plan offerings for the new year other than the addition of one Blue Cross Blue Shield medical plan!
Upon selection, please fill out an election form and return to your manager for processing to assure you are enrolled properly come December 1, 2022.
Eligibility and Qualifying Life Events
You are eligible to participate in benefits if you are full-time and work a minimum of 30 hours per week. Your coverage is effective the 31st day following your date of hire.
You may also enroll eligible dependents for benefits. A dependent is a legal spouse or child of the plan participant.
Dependent children are eligible for medical, dental and vision until the last day of the calendar year the child turns 26.
If you separate from employment, COBRA continuation of coverage may be available as applicable by law.
DHD2 offers PPO 4000 and PPO 2000 plans through Blue Cross Blue Shield of Michigan. Your contributions vary based on if you are a single, two person, or family contract.

Livongo Diabetes Care
OBlue Cross offers diabetes management through Livongo. This includes a smart glucometer that automatically uploads blood sugar readings, making logbooks a thing of the past. Unlimited lancets and test strips are delivered right to your door as well as 24/7 one-on-one coaching. All available in the comfort of your home!
