D'Addario Annual Open Enrollment Information 2022

Open Enrollment is your opportunity to learn about the 2022 Benefits Program, view or update your current coverage options or elect new coverage options for you and your family. You can access and review our 2022 Benefits Guides in "Additional Resources" below. Employees can attend our upcoming educational meetings to learn more about our 2022 benefits programs.

Benefits Overview

BENEFITS CHANGES THIS YEAR

UnitedHealthcare Medical PPO and EPO Plans

  • The deductible will no longer apply to In-Network Lab and X-Ray services
  • Specialist copays are increasing from $40 to $50
  • Tiers 1, 2, and 3 Rx copays are increasing from $25 / $50 / $75 to $25 / $75 / $100
  • Tiers 1, 2, and 3 Rx Mail Order copays are increasing from $25 / $50 / $75 to $62.50 / $187.50 / $250
  • Employee contributions for the Under $50,000 tier of the UnitedHealthcare Medical EPO Plan are remaining flat


Kaiser HMO Medical Plan

  • The maximum copay for specialty drugs is increasing from $200 to $250


Aetna Dental Plan

  • The Aetna Dental DMO plan will now include Orthodontia coverage for adults and dependent children (up to age 18)
  • The Aetna Dental PPO Plan 1 and PPO Plan 2 annual plan maximums are increasing from $1,500 to $2,000
  • In addition to these plan enhancements, your employee contributions are decreasing!


Guardian Dental

  • There will be no plan or rate changes to the plan this year


Davis Vision

  • There will be no plan or rate changes to the plan this year

How To Enroll

How to Enroll for the 2022 Plan Year
  • Visit ADP WorkforceNow to review your current coverage and enroll and modify your elections for 2022
  • If you'd like to change your existing coverage or make new elections, you must do so by June 17th. Your current medical, dental and vision elections will carry forward to 2022 if you don't make a new election

We are pleased to introduce a new service to all employees participating in our Medical plan: Benefits Concierge. This is a valuable resource provided to us by our benefits consulting firm, NFP. They provide a one-stop contact center for answering your medical plan questions and resolving medical claim issues.
