What's New for 2023?

There will be NO changes to your plans or benefit offerings for 2023!


Benefits Overview

Medical/Prescription: Excellus BCBS will continue as your medical carrier with the following plan designs:


Deductible: $3,000 single/ $6,000 family

Coinsurance: 90% plan pays (Employee cost share 10%)

Out of Pocket: $4,000 single/$8,000 family 


Telemedicine: Telemedicine will be provided through Excellus BCBS. 


Dental: Provided though Lincoln with no plan changes.


Vision: Provided through Reliance Standard with no plan changes.


Basic and Supplemental Life/AD&D:  Provided through Reliance Standard with no plan changes. 


Short-Term, Long-Term Disability and paid family leave through State of NYProvided through Reliance Standard with no changes.


Wellness: Vitality will continue as your wellness platform.


Health Savings Account (HSA): HSA Bank.

EMPLOYEE ELIGIBILITY: If you do not enroll during the Open Enrollment period, you must wait until the next Open Enrollment period unless you experience a Qualifying Life Event. If you experience a Qualifying Life Event (i.e., birth or adoption of a child, marriage, divorce, loss of other coverage, etc.) you must enroll within 30 days of the Qualifying Event.

 

DEPENDENT ELIGIBILITY: You may also enroll eligible dependents for benefits coverage. A ‘dependent’ is defined as the legal spouse and/or ‘dependent child(ren)’ of the plan participant or the spouse.

  • Your Legal Spouse
  • Your Dependent Children: Children are eligible up to the end of the month of 26th birthday regardless of marital or student status.


What is a Qualifying Life Event?

• Marriage

• Divorce

• Birth/Adoption of a child

• Death of a spouse or other enrolled dependent

• Change in spouse’s benefits or employment status

• A dependent becomes eligible for Medicare or Medicaid

 

If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.

Enrollment with EASE

ENROLLMENT INSTRUCTIONS:

1.You will receive an Email with a link to Ease. Click the blue “login” button with the email message to be directed to the enrollment portal.

2.Enter your User ID and password

3.Follow instructions and enroll in your benefits

4.Make sure to sign your forms, save your elections and print your confirmation statement.

Medical

Excellus BCBS will continue as your medical carrier for 2023.


When you utilize In Network providers you receive higher benefits. Employees who seek care from a provider that is Out of Network will be responsible for higher costs.

Telemedicine

Telemedicine with Excellus is powered through MDLive. This is available to you and your eligible dependents 24 hours a day, 7 days a week.


When can I use MDLive?

  • Your doctor isn't available
  • You become ill when traveling
  • You are considering a hospital visit for a non-emergency health condition.


How do I prepare for a visit?

  • Be sure to check that your internet, wi-fi and phone connections will allow you to access your doctor's online portal or mobile site.
  • Vital signs such as weight, blood pressure, body temperature may be needed
  • Signs and symptoms that you have been experiencing and for how long
  • Previous medical history may be needed. Your consent is typically required and may be requested through the online portal or mobile site. Consent can be verbal or written.

Understanding an HSA

A Health Savings Account (HSA) is a tax-free savings account is owned by you, is 100% vested from day one, and let’s you build up savings for future needs. The funds may be used to pay for qualifying healthcare expenses not covered by insurance or any other plan for yourself, your spouse, or tax dependents. You decide how much you would like to contribute, when and how to spend the money on eligible expenses, and how to invest the balance.


To have and HSA you must meet several basic qualifications.

  • To be eligible to open and contribute to an HSA, you must have coverage under a qualified High Deductible Health Plan (HDHP).
  • Participants cannot be covered by any other health insurance plan that is a non-HDHP with a HSA, i.e., cannot be enrolled in spouses plan that is a PPO (this exclusion does not apply to certain other types of insurance, such as dental, vision, disability or long-term care coverage).
  • Participants cannot participate in a Healthcare FSA or spouse/domestic partner’s Healthcare FSA or Health Reimbursement Account (HRA).
  • Participants cannot be enrolled in Medicare or Medicaid.
  • You cannot be eligible to be claimed as a dependent on someone else’s tax return.
  • You have not received Department of Veterans Affairs Medical benefits in the past 90 days, unless the Veteran has a disability rating. (There may be additional special circumstances, check with your tax preparer).


Each year the IRS places a limit on the maximum contribution amount that can be added to your HSA account. For 2023 the contribution limits are,

  • Employee Only - $3,850
  • Two-Person/Family - $7,750

Flexible Spending Account

Flexible Spending Accounts (FSA) allow you to reduce your taxable income by setting aside pre-tax dollars from each paycheck to pay for eligible out-of-pocket health care and dependent care expenses* for yourself, your spouse and your dependent children. 


In order to participate in the FSA, you must enroll each year. Your annual contribution stays in effect during the entire year (January 1st through December 31st). The only time you can change your election is during the enrollment period or if you experience a change-in-status event. Also, you must elect this benefit within 30 days of your hire date or first date of benefits eligibility.


Limited Purpose FSA vs. Dependent Care FSA


LIMITED PURPOSE FSA

If you open or contribute to a Health Saving Account (HSA), you may only enroll in a Limited Purpose FSA.

If you enroll in a HDHP (High Deductible Health Plan) and elect a Health FSA, you will automatically be enrolled in the Limited Purpose FSA.

A limited purpose FSA will reimburse you for dental and vision expenses, but you cannot claim the same expense on both the FSA and HSA accounts. 


DEPENDENT CARE FSA

A Dependent Care FSA allows you to pay for eligible dependent care expenses with tax-free dollars so that you and your spouse can work or attend school FT.

Unlike the Health Care FSA, funds in a Dependent Care FSA are only available once they have been deposited into your account and you cannot use the funds ahead of time.

  • You may set aside up to $5,000 annually in pre-tax dollars, or $2,500 if you are married and file taxes separately from your spouse.
  • If you participate in a Dependent Care FSA, you cannot apply the same expenses for a dependent care tax credit when you file your income taxes.

Lincoln will continue as your dental carrier with no plan changes.


Your Lincoln dental coverage is provided at no cost to you. You have the freedom to select the dentist of your choice; however, when you visit a participating PPO or Premier dentist, you will have lower out-of-pocket costs, no balance billing, and claims will be submitted by your dentist on your behalf. Out of network dentists may bill you for the difference between the contracted rate and the dentist’s fee (balance billing).

Vision

Reliance Standard will continue as your Vision carrier with no plan changes.


Under this plan, you may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services from an out-of-network provider, you will be required to pay that provider at the time of service and submit a claim form for reimbursement. Your vision network is EyeMed Insight.

Basic Life and AD&D

Basic Life Summary

Reliance Standard will continue to provide your Life and AD&D benefits with no plan changes.


Life insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. AD&D insurance is equal to your Life benefit in the event of your death being a result of an accident and may also pay benefits for certain injuries sustained.


Your Basic Life Benefit

  • 100% company paid benefit, no cost to you
  • Crucible Industries provides 2x base annual earnings up to $400,000
  • Portable: Continue your term insurance if your employment ends
  • Convertible: To individual life policy when coverage ends under the plan

Voluntary Life and AD&D

Reliance Standard will continue to provide your Voluntary Life and AD&D benefits with no plan changes.


Employees have the opportunity to enroll in Supplemental Life insurance. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage. Coverage is also available for your spouse and/or child dependents. It is typically required that you elect coverage for yourself in order to be eligible for coverage on your dependents.


Your Voluntary Life Benefit

  • Available to active full-time employees
  • 100% employee paid benefit
  • Up to the lesser of 5x your salary or $500,000
  • Spouses and children may also be eligible


It is important to name your beneficiary. A beneficiary is the person who will receive your life insurance benefit in the event of your death. You should review your beneficiary elections on a regular basis to ensure they are updated as life changes.

Short Term Disability

Reliance Standard will continue to provide your Short-Term Disability benefits with no plan changes.


Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. 

Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.


Your Short-Term Disability Benefit:

  • 100% employer paid benefit
  • Payments last up to 26 weeks
  • Covers 70% of your weekly income, up to $2000 per week
  • Begins on the first day of an accident or the eighth day of an illness

Similar to Short-Term Disability, Long-Term disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.


Your Short-Term Disability Benefit:

  • 100% employer paid benefit
  • Payments last as long as you are disabled
  • Covers 60% of your monthly income, up to $8000 per week
  • Begins after 180 days.

Employee Assistance Program

Your employee assistance program is through HelpPeople. Services through the HelpPeople EAP are confidential and available to you and your dependents 24 hours a day 7 days a week. 

Day and evening appointments are available at their locations in Syracuse, Liverpool and Fulton.

Paid Family Leave

Paid Family Leave (PFL) will provide time off so an employee can:

  • Bond with a newly born, adopted, or fostered child,
  • Care for a family member (spouse, domestic partner, child, parent, parent-in-law, grandparent, or grandchild) with a serious health condition, or assist loved ones when a family member (spouse, domestic partner, child or parent) is deployed abroad on active military duty.

 

Additional Protections under PFL

  • Employees have a right to return to the same or comparable job upon return from Paid Family Leave.
  • Employees are guaranteed continued health insurance while on leave. Employers may require employees to continue to pay their health insurance premium contributions.
  • Citizenship and immigration status do not impact eligibility.
  • Employers cannot discriminate against employees for taking Paid Family Leave.
  • The program is mandatory for all private employers to provide for all NY covered employees.

 

Eligibility under PFL

  • Employees with a regular work schedule of 20 or more hours per week are eligible after 26 consecutive weeks of employment preceding the first full day of leave.
  • Employees with a regular work schedule of less than 20 hours per week are eligible after 175 days worked preceding the first full day of leave.


How to File a PFL Claim

  • Employee must notify their employer 30 days prior to a leave, when practical.
  • Employee must complete an NY PFL claim form available from HR. 
  • Employee needs to obtain supporting documentation for a leave (i.e. birth certificate, military deployment, certification, etc.) and submit with claim form.
  • Employee submits claim form, along with supporting documentation, to First Reliance Standard, which will process and pay or deny the claim within 18 days of receipt of completed claim.
