Introduction

Helping you and your family reach and maintain good health is very important. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


How To Enroll

Action Required

Remember: The benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.

You can, however, modify your elections under certain circumstances, called "Qualifying Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child. Refer to your Employee Benefits Guide or contact Human Resources if you have questions about qualifying events.


  • If you'd like to change your existing coverage or make new elections, you must do so by September 9, 2023. Your current medical, dental and vision elections will carry forward to the new plan year if you don't make a new election.
  • To participate in a Flexible Spending Account in 2023, you must make an active election, even if you're already participating in 2022.

Medical

Crest Foods offers you three medical plan options through Blue Cross Blue Shield of Oklahoma (BCBSOK): the $3,000 Deductible Plan, the $1,500 Deductible Plan, and the $1,000 Deductible Plan. All three of our plans cover in-network preventative care services at 100% and provide coverage both inside and outside of the BCBSOK network


Everyone's needs change over time, so it's important to review your medical plan options each year. While the plans are similar in some ways, each has its own features when it comes to how much you pay out of pocket for medical and prescription expenses. The Ameritas vision plan provides you and your family with quality vision benefits and materials like glasses and contacts at an affordable cost. You can visit any licensed vision VSP Network specialist and receive coverage, but your benefit dollars will go further when you stay in network.

Employer-Paid Basic Life and Accidental Death & Dismemberment (AD&D)


Crest Foods provides eligible employees with company-paid basic life and AD&D insurance. The benefit is a flat amount based on your eligibility class. In addition to the life insurance benefit, an additional Accidental Death & Dismemberment benefit is payable in the event of death, loss of limbs or loss of eye sight, speech or hearing as the result of an accident.


Voluntary Life and (AD&D)


In addition to the company-paid life and AD&D benefit that Crest Foods provides, you have the option to purchase additional coverage under the Voluntary Life and AD&D plan. You must purchase coverage for yourself in order to purchase coverage for your dependents.
