ELIGIBILITY - All Full-Time employees (Civitas Senior Living, Senior by Design, Front Porch Creative, and community-based employees in the following roles: Executive Director, Wellness Director, Memory Care Director, Community Relations Director, Community Outreach Director, Food and Beverage Director, Chef Manager, Director of Resident Engagement, and Business Office Manager) are eligible to participate in the following benefit programs on the first day of the month following 30 days of employment. Employees who are eligible to participate in benefit programs may also enroll their dependents in the medical/prescription drug, dental, and voluntary benefits programs. For the purpose of our benefit plans, your dependents are defined as follows:
