ELIGIBILITY - All full time employees working 30 hours or more per week are eligible. For newly hired employees, after you satisfy your 30 day orientation period, you are eligible for coverage first of the month following 60 days of service. For the purpose of our benefit plans, your dependents are defined as follows:

Your spouse*
Your children who are under the age of 26, regardless of marital or student status
Your children over age 26 who are mentally or physically disabled and dependent upon you for support (proof of condition and dependence must be submitted)
Your children who are covered by a Qualified Medical Child Support Order
