ELIGIBILITY - All full time employees working 30 hours or more per week are eligible. For newly hired employees, after you satisfy your 30 day orientation period, you are eligible for coverage first of the month following 60 days of service. For the purpose of our benefit plans, your dependents are defined as follows:

User ID: First Initial of your First Name + your Full Last Name + last 4 digits of your Social Security Number + 692. Your Initial Password: Last 4 digits of your Social Security Number (SSN).
