2022 Benefits Open Enrollment

Open Enrollment is your opportunity to evaluate your current benefit elections and make changes for the new plan year, beginning September 1st. Meetings will be conducted to discuss updates to your benefits package and to assist you with any questions or changes you may need to make.

 

These meetings will last approximately one hour and 15 minutes, so please plan accordingly. Remember to bring information necessary to add dependents, such as birth dates and social security numbers. 


Open Enrollment paperwork is due to HR by August 5th.

Changes This Year

We're excited to announce some exciting changes, this year!


Prescription Drug Formulary

BCBS will be moving your pharmacy benefit to a more cost-effective and clinically appropriate managed drug list.


Member notices and support: Members may be impacted if using prescription medications that are no longer covered, using prescriptions that may still be covered but at a higher payment tier (non-preferred) or may be subject to new utilization management program criteria before prescription drug benefit coverage may be approved. Impacted members may receive a communication alerting them of the change. The letter will explain how the change may impact them. Members will be encouraged to talk with their health care provider(s) about their prescribed medications and if any changes in their drug therapy may be right for them.


Livongo Programs ($0 Cost to You)

Diabetes Management Program

This program is provided to you and your family members with diabetes and coverage through BCBSOK. Livongo helps 700,000+ members worry less about managing diabetes. With Livongo, you’ll get a smart blood glucose meter to guide your journey, a connected app that tracks numbers so you don’t have to, and access to expert coaches for advice on diet, lifestyle and more, all at no cost to you!! This program is available beginning April 1, 2022, but you have to enroll by visiting join.livongo.com/WELL-BCBSOK/hi or call 800-945-4355 and use registration code: WELL-BCBSOK.


High Blood Pressure Management

This program is provided to you and your family members with high blood pressure and coverage through BCBSOK. This program provides a free advanced blood pressure monitor, personalized insights, one-on-one coaching, easy-to-use app and dashboard and guidance on healthy habits, all at no cost to you!! This program is available beginning April 1, 2022, but you have to enroll by visiting ready.livongo.com/BCBSOK-HEALTH/hi or call 800-945-4355 and use registration code: BCBSOK-HEALTH. 


Wondr ($0 Cost to You)

Metabolic Syndrome Reversal Program

Wondr is a behavioral counseling program for metabolic syndrome reversal and weight management. Now you can lose weight, gain energy, sleep better, and improve your mind and body—all while eating your favorite foods.


Dental

We are increasing your dental benefit max from $2,500 to $3,000. We are also excited to extend the orthodontic benefit to enrolled adults. The Adult Ortho Benefit, AND additional $500 Annual Max Benefit, will go into effect on January 1. 


Vison

We are making a few enhancements to our vision plan. The Annual Exam copay is reducing from $20 to $10, and the frame/contacts allowance is increasing from $150 to $200 ($220 for featured brands – frames only).


Basic Life

We are increasing the employer paid Basic Life/AD&D benefit from $30,000 to $40,000.


Voluntary Life

We are increasing the benefit max, for your spouse, from 50% of your benefit amount, to $100%.


Guaranteed Issue!!

We were able to get a “TRUE” open enrollment for your Voluntary Life/AD&D, STD and LTD benefits, for this year only. This means that if you previously declined these benefits and wish to enroll, or increase your current benefit amounts, during this open enrollment, you can do so without having to prove insurability. This is your ONLY opportunity to elect these benefits without having to prove insurability. If you decline this benefit, and wish to enroll during a future open enrollment, you will be required to provide evidence of insurability (EOI) and may be denied coverage.

How To Enroll and Forms

Be sure to attend one of the in-person presentations to get your enrollment packet.


Your benefits packet includes your personal Benefits Statement, which reflects your current benefit elections and payroll deductions, and your customized Benefits Application/Change form, which also includes your current benefit elections. Please review for accuracy, and report any discrepancies to HR.



Action Required:

Everyone is required to complete a new Benefits Enrollment/Change form, even if you aren’t making any changes to your current benefit elections. This form is included in your benefits packet. Please review for accuracy and make any necessary changes, then mark your new benefit elections (either continuing your current benefit elections or making a change), and return to HR no later than August 5th at 4:00 p.m. 


New employees will need to complete the Benefits Enrollment Form that is included in their packet. Blank forms are also available, below.

As your employer, we automatically provide a certain level of life insurance coverage for you and give you the opportunity to purchase additional coverage for you and your dependents. 


Employees who wish to supplement their basic life insurance benefit, may purchase additional coverage.


The Supplemental Life/AD&D, Short-term Disability and Long-term Disability benefits are offered on a voluntary basis.


Symetra has agreed to allow members who previously declined these benefits, to enroll during this open enrollment, without having to provide evidence of insurability. This is your ONLY opportunity to enroll in these benefits without having to prove insurability.
