Eligibility
Employees working at least 30 hours per week are eligible to participate in benefits offered by ColdSpark Media. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

If you are newly hired, you are eligible for benefits after 60 days of employment. You must complete the enrollment process through the EASE on-line enrollment platform in order to be set up with the offered insurance coverages.
You can modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

UPMC HealthPlan will continue to administer your medical and prescription drug benefits this year.
If you live in the Western PA Area, you will be eligible to enroll with the Pittsburgh based, the MyCare EPO $1250/$2500 Policy.
If you live outside of Western PA, you will be eligible to enroll with the National based plan, the PPO $2000 Policy. UPMC HP partners with the Super Med Network for Ohio residents, and the CIGNA Network for all other states.

ColdSpark Media's dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Ameritas network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your out of pocket costs will be higher.

Children that are covered under the medical plan are automatically also covered under the Pediatric coverage with UPMC HP.
There is an option to use $100 of the annual benefit towards vision expenses. You must complete the claim form and submit along with your receipt for the reimbursement.

ColdSpark Media offers vision coverage through UPMC HP to help pay for eye exams and contacts or glasses. You receive a higher level of benefits when you see a provider in the UPMC HP Vision Network. Review your coverage details in your Benefit Guide.

Note, there is also a $100 allowance towards Vision coverage needs included with the Ameritas Dental Plan. You must file the claim form along with the receipt in order to be reimbursed.
ColdSpark Media provides Short-Term Disability coverage through UNUM.
Short Term Disability
