Medical/RX: Provided through Medical Mutual (2 plan offerings)
•Plan 1 - 100% HSA PPO: $3,000/$6,000 deductible
•Plan 2 - 80% PPO: $250/$750 deductible
*NEW* DENTAL: Provided through Mutual of Omaha
*NEW* VISION: Provided through Mutual of Omaha
Health Savings Account:
If you enroll in the 3000 HSA you have the option to open a health savings account with the bank of your choice. Once the account is set up, please provide the account information to HR so the employer contribution and pre taxed employee contributions can be set up accordingly.
How to enroll for OPEN ENROLLMENT - Effective July 1, 2022:
If you are enrolling or waiving coverage during open enrollment, we ask you complete the election form located in the Benefit Alert Newsletter (pg. 19) and return to HR no later than Friday June 17th
How to enroll if you are a NEW HIRE:
If you are a New Hire of Cleveland Menu you have a 90-day waiting period from your full-time date of hire. Within your new hire waiting period please review all benefits being offered and complete the election form in the Benefit Alert Newsletter (pg. 19). Even if you are waiving benefits, please complete the election form.
For the medical, dental, and vision you are able to pick which coverages you would like to enroll in. For example, if you waive medical coverage but want to elect dental you are able to do so.
The election form needs to be completed within the 90 days and returned to HR
