WELCOME

Helping you and your family reach and maintain good health is very important to CLAW. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

OVERVIEW

CLAW is committed to providing a competitive, cost-effective benefit program. Below are the benefit offerings effective 1/1/2023-12/31/2023:


  • Medical/Rx Insurance is still offered through HAP
  • The 3 Medical plan options are available:
  • PPO $1000
  • PPO $3000
  • PPO HSA $2500
  • All three medical plan options have decreased in employee contribution
  • If electing the HSA plan, your Health Savings Account will be through Health Equity
  • Dental coverage now offered through Mutual of Omaha
  • Vision coverage still offered through EyeMed
  • Life, Disability and Voluntary disability coverage is now offered through Prudential

  2. Initial enrollment and life events can be performed on (ADP) workforcenow.adp.com or the mobile app.


Remember: The benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.    

You can, however, modify your elections under certain circumstances, called a “Qualifying Life Event”. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child. HAP

CLAW offers eligible employees three plan options. All three plans are PPOs and one of them has a Health Savings Account (HSA) which includes employer matching funds. This is just an overview, for a more detailed look, please view the attached Benefit Summary.


Please note: With the PPO HSA $2500, you are responsible to pay the entire deductible before you go into coinsurance (also known as cost sharing, where you pay 20% and HAP pays 80% of the expense). You can pay for expenses using a Health Savings Account (HSA) which will save you from paying taxes on any amount in this account. When you elect this account, you will automatically get set up with a HealthEquity account, they will mail you a welcome packet and debit card - this is the card you use to pay expenses.

HEALTH SAVINGS ACCOUNT

Health Equity

A Health Savings Account (HSA) is a tax-free savings account that is owned by you, is 100% vested from day one, and lets you build a savings for future medical expenses. The funds in this account are meant to be used to pay for the entire deductible and once that's met you can continue to use the money for coinsurance costs. For example, you have a $2,500 deductible - this is the amount you will pay out of pocket for all medical visits, prescription drugs, and all other covered medical procedures. In this case, it would be wise to elect to contribute $2,000 annually to your HSA to pay for medical expenses and CLAW will contribute up to $500 to your account (that's free money!), but you must contribute at least $500 to receive a match of $500, if you contribute only $250, you will only receive $250.

?HSA Reminders:

  • Money in your HSA account earns tax-free interest.
  • Any unused HSA funds roll over to the next year.
  • You can spend your HSA dollars on eligible health care expenses or save and investment them for the future.
  • You decide when and how to spend or save the money in your HSA. The money is yours forever. It doesn’t expire, and you can take it with you if you change jobs or switch to another high-deductible health plan.
  • You can invest a portion of your HSA balance in mutual funds, stocks and bonds. Generally, this option is available when your balance reaches $2,000.
  • You can use your HSA to save for retirement. At age 65, you can use the funds for any purpose without a penalty. The money you take out to pay for eligible health care expenses continues to be tax free. You also can take money out for other reasons without paying a penalty.

VIRTUAL VISITS

HAP AmWell

CLAW provides those employees who are covered by one of the HAP medical plans access to Telehealth visits with AmWell. Telehealth provides members the opportunity to receive medical and mental health care from the comfort of your own home. With Telehealth, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.


How Much Does it Cost?

  • PPO Plans (Non-HDHP/HSA): $30 copay
  • HDHP/HSA Plan: 20% after deductible.
  • Prescriptions: If prescribed during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.

Mutual of Omaha

Our dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Mutual of Omaha network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher.

LIFE & DISABILITY

PRUDENTIAL

We provide all eligible team members with Company paid Basic Life/AD&D coverage. Voluntary Employee Life, Dependent Life, Short-Term Disability and Long-Term Disability are available as Employee paid benefits through Prudential.


Basic Life/AD&D

Employees working 30 or more hours per week are eligible for this employer paid coverage payable to your designated beneficiary in the event of your death. An additional Accidental Death & Dismemberment (AD&D) benefit is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.


Voluntary Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Voluntary Life/Dependent Life and AD&D insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short-Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short-Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage through Prudential pays 60%of your income to a maximum of $1,500 per week for up to 13 weeks, benefits for illness or accident begin on the 8th day.


Long-Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long-Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a 90-day elimination period are equal to 50% of covered monthly earnings up to $5,000 for as long as you are disable or your Social Security Retirement age, whichever is sooner.
