Helping you and your family reach and maintain good health is very important to National Hospitality Services. National Hospitality Services is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Eligibility
If you are an active full-time employee and work 30 hours or more per week, you are eligible for the Medical Plan. If you are a regular full time employee working 32 hours or more you are eligible to participate in the dental, vision, life and AD&D and voluntary benefits provided by UHC.
In addition, the following family members are eligible for certain benefits as well:
• Legally married spouse or domestic partner
• Dependent child(ren) up to age 26
• Dependent Disabled child(ren)
Qualifying Events
You can change your benefit election during the year if you have a qualifying life event. Qualifying life events include, but are not limited to:
• Marriage, divorce or legal separation
• Birth or adoption of a child
• Change in child’s dependent status
• Death of a spouse, child or other qualified dependent
• Change in employment status or a change in coverage under another employer-sponsored plan
If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.
During your enrollment window you will make decisions on all of your benefits through National Hospitality Services. This is the only time that you can make changes unless you have a qualifying life event.
When you have reviewed the enrollment materials and are ready to make your 2023 benefit elections, you can do so through UKG.

UnitedHealthcare will continue to administers your medical and prescription drug benefits this year. The medical plan includes prescription drug benefit, which covers prescriptions at participating pharmacies and mail-order maintenance drugs.

National Hospitality Services dental plan provides comprehensive coverage to help you and your family maintain good dental health. With this plan, you can see any dentist you want, anywhere across the country. When you choose a dentist who is part of your plan’s large national network, you may receive discounted rates only available to members.

National Hospitality Services offers vision coverage through UnitedHealthcare to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the UnitedHealthcare Network. Review your coverage details in your Benefit Guide and Summary attached below.

Health Savings Accounts (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money. To be eligible for an HSA, you must be enrolled in a High Deductible Health Plan (HDHP).

NHS matches $1 for $1 up to $55 per month.
UnitedHealthcare Motion
You also have the opportunity to earn HSA dollars with the UnitedHealthcare Motion Program. Taking steps to a healthier you and earning up to $3/day or $1,095/year in your HSA

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.

How the Plan Works
You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until March 31st annually to file your claims. If you do not use up all the Traditional/Limited FSA dollars you elected to have deducted for the year, you will be able to carry over up to $610 from the current plan year into the new plan year. However, FSA Dependent Care still has the “use it or lose it” rule and unspent dollars cannot be carried over into the next plan year.
National Hospitality Services provides eligible employees with Basic & Voluntary Life/AD&D coverage as well as Voluntary Short Term Disability and Long-Term Disability through UnitedHealthcare. Please see HR to confirm if you are eligible for these benefits.
Basic Life/AD&D
Life insurance can help provide for your loved ones if something were to happen to you. National Hospitality provides full-time eligible employees with $30,000 in group life and accidental death and dismemberment (AD&D) insurance through UnitedHealthcare.
Voluntary Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Voluntary Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Voluntary Short Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. You have the option to enroll in Short Term Disability coverage through UnitedHealthcare. Benefits begin after a 14 day waiting period. The benefit pays 60% of your income to a maximum of $1,000 per week.
Long Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a 180 day elimination period are are equal to 60% of covered monthly earnings up to $5,000 for full-time employees.
