Tobacco Free Incentive Affidavit

In order to improve the health and well-being of our employees, while also maintaining a high level of coverage at an affordable rate, The City of Painesville has a Wellness Program for Tobacco Cessation.


With this program all employees covered under the medical plan who use tobacco will be charged an additional health insurance premium. Additionally, any eligible spouses covered under the plan who use tobacco will also be charged an additional health insurance premium.


RANDOM testing will occur:

  • A positive test resulting from any tobacco cessation medication (gum or patches included) will not cause an employee to be responsible for the additional fee per month.
  • Employees and spouses can be randomly generated each quarter to be tested for nicotine
  • A failed test, unless noted above, will result in immediate change to tobacco user status, and that employee and/or spouse will be back charged $20 per month penalty to the beginning of that calendar year.


Enroll or Qualifying Event Change

Full-Time Employees (minimum of 30 hours/week)

*Employees are eligible the first of the month following your date of hire. If you were hired on the first of the month, then your benefits are effective on the date of hire.

 

EMPLOYEE ELIGIBILITY: If you do not enroll during this Open Enrollment period, you must wait until the next Open Enrollment period unless you experience a Qualifying Life Event. If you experience a Qualifying Life Event (i.e., birth or adoption of a child, marriage, divorce, loss of other coverage, etc.) you must enroll within 30 days of the Qualifying Event.

 

DEPENDENT ELIGIBILITY: You may also enroll eligible dependents for benefits coverage. A ‘dependent’ is defined as the legal spouse and/or ‘dependent child(ren)’ of the plan participant or the spouse.

  • Your Legal Spouse: If your legal spouse has coverage at his/her employer, they are not eligible for the City of Kirtland per the spousal waiver policy. You must submit a copy of your marriage license, if you have submitted documentation previously, you do not need to resubmit.
  • Your Dependent Children: Children are eligible up to the end of the month of 26th birthday regardless of marital or student status. You must submit a copy of each child's birth certificate, if you have submitted documentation previously, you do not need to resubmit.

 

What is a Qualifying Life Event?

• Marriage

• Divorce

• Birth/Adoption of a child

• Death of a spouse or other enrolled dependent

• Change in spouse’s benefits or employment status

• A dependent becomes eligible for Medicare or Medicaid

 

If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.

When you utilize providers that are in MMO’s network to receive higher benefits. Employees who seek care from a provider that is not in MMO’s network, will be responsible for higher costs. 


Your prescription drug coverage is through Caremark. You can receive generic medications for hypertension, diabetes, cholesterol, and asthma at $0 copay. If you are on a maintenance or long-term medication, then you must fill these prescriptions through mail order. You can receive a 90-day supply at any CVS Pharmacy.

Flexible Spending Account (FSA)

Health FSA with Dependent Care Kit

Health FSA with Dependent Care Kit

Flexible Spending Accounts (FSA) allow you to reduce your taxable income by setting aside pre-tax dollars from each paycheck to pay for eligible out-of-pocket health care and dependent care expenses* for yourself, your spouse and your dependent children.


In order to participate in the FSA, you must enroll each year. Your annual contribution stays in effect during the entire year (January 1st through December 31st). The only time you can change your election is during the enrollment period or if you experience a change-in-status event. Also, you must elect this benefit within 30 days of your hire date or first date of benefits eligibility.


HEALTH CARE FSA ROLLOVER

Health Care FSA’s have a $500 roll over feature, which allows any amount of $500 or less remaining in your account at the end of the plan year to roll over into the new plan year.

At the end of each plan year, you may use these funds left over from the current year FSA balance to pay for eligible expenses incurred in the first 2 ½ months of the following year. Any unused funds beyond the 2 ½ month grace period will be forfeited.

FSA Plan Year runs from January 1 to December 31

Contributions for the FSA plans – both the FSA and Dependent Care FSA – are taken out in 26 pay period. 





Dental

Guardian remains the carrier for your dental benefits.


You may go to any dentist. However, those who belong to the DentalGuard Preferred network will be most cost effective.

You may go to any eyecare provider. However, those who belong to Davis or VSP networks will be most cost effective.

  • Exams: Once every 12 months
  • Lenses: Once every 12 months

  • Contacts: Once every 12 months (contacts in lieu of frames/lenses)
